    • An assembly for occluding a vascular site (e.g., an aneurysm) of a human or veterinary patient includes a vaso-occlusive member, a pusher member having a distal end and a severable junction located proximal to the distal end, and a pivotable coupling that couples the pusher member to the occlusive member. A delivery catheter can be used to deliver the vaso-occlusive member to the vascular site. A method of using the assembly to occlude an aneurysm having an aneurysmal sac and an aneurysmal neck, comprises locating the catheter within the aneurysmal neck, and manipulating the pusher member to place the vaso-occlusive member within the aneurysmal sac. The method further comprises severing the severable junction to detach the vaso-occlusive member from the pusher member. As a result, an axial force is applied by the vaso-occlusive member in a proximal direction, which buckles the pivotable coupling to laterally deflect the axial force. The lateral deflection of the axial force caused by the buckling of the pivotable coupling prevents the catheter from being displaced from the aneurysmal neck by the axial force.

    • A dilatation catheter assembly which has a dilatation catheter with a sheath mounted on the catheter shaft having an inner lumen extending in the distal portion of the sheath which is configured to slidably receive at least a portion of the dilatation balloon on the distal end of the catheter. The position of the sheath is fixed with respect to the catheter, e.g. a friction fit or bonded in some manner, before the assembly is inserted into the patient's vasculature with the distal portion of the sheath covering a portion of the balloon but leaving a portion of the balloon uncovered with a length equal to the stenotic region to be dilated so that the entire stenosis can be dilated at the same time. The distal portion of the sheath is preferably formed of a flexible polymeric tube with inelastic reinforcement to prevent substantial expansion thereof when the balloon is inflated.

    • A guidewire replacement device having a proximal shaft section, a distal shaft section and a flexible intermediate shaft section which facilitate the articulation of the distal shaft section with respect to the proximal shaft section. The intermediate shaft section has as aperture for the egress or exit of an in-place guidewire during a guidewire exchange. The proximal end of an in-place guidewire is inserted through the distal guidewire port in the distal end of the distal shaft section, through the distal section and out the aperture in the intermediate shaft section. The exchange device is advanced through the inner lumen of a guiding catheter over the in-place guidewire until the distal end of the exchange device extends through the proximal guidewire port of a rapid exchange type intraluminal catheter. Once the guidewire exchange device is properly placed, the in-place guidewire can then be withdrawn from the patient. The replacement guidewire can then be advanced through the inner lumen of the proximal shaft section, the guidewire passageway of the intermediate section, the inner lumen of the distal section into the guidewire receiving inner lumen of the intraluminal catheter. The guidewire exchange device may then be removed from the patient.
