摘要： A surgical instrument is provided for use in endoscopic or laparoscopic procedures. The instrument includes a handle portion, an endoscopic portion extending from the handle portion, an articulating section pivotably connected to a distal end portion of the endoscopic portion, and a retractor assembly operatively associated with the articulating section. Structure is provided for manipulating the articulating section relative to the longitudinal axis of the endoscopic portion within an angular degree of rotation. An injection port may also be provided to deliver fluids through the endoscopic portion to the operative site.

The instrument includes a handle assembly having a pivoting handle and a stationary handle, a barrel portion to which an elongated body assembly is secured, and a locking mechanism for releasably positioning a tool mechanism located at the distal end of the body assembly. In one embodiment the locking mechanism comprises a trigger, a brake and a latch spring. In another embodiment the locking mechanism comprises a latch spring and a locking block. A rotational knob and locking member may also be provided to position the body assembly at various orientations to the longitudinal axis.

摘要： A tissue gripping device for use with a cannula. The device includes a housing member which has a generally cylindrical shape and is provided with a plurality of slots about its circumference. A cannula locking collet and collar are provided along with a tissue gripping member so that the entire device fits over a cannula and is secured thereto. Once the cannula is positioned in the body wall of a patient, the tissue gripping member is moved towards the housing member so that a plurality of finger-like expansion members protrude through the slots in the housing member to anchor the tissue gripping device and the cannula in place for a surgical procedure.

The piercing tip is maintained in the exposed position by a latching mechanism associated with the obturator shaft, and is automatically withdrawn into the cannula under the force of a spring when the proximal force is removed from the cannula, the cannula thus advancing distally and releasing the latching mechanism. 摘要： A tissue retaining ring for use with a plurality of surgical fasteners mounted within a disposable loading unit utilized during a vascular anastomosis inlcudes a genrally circular, semi-pliable material having a series of alternating loops and arcuate portions. Each of the loops defines an aperture therein which is dimensioned to receive a distal end of one of the plurality of surgical fasteners. When the instrument is fired, the surgical fasteners are simultaneously deformed through the tissue ring and through the tissue. The tissue retaining ring holds the tissue in place along the surgical fastener and reduces the chances of tissue slippage after deformation.

摘要： Apparatus and method for surgery is disclosed which includes a retractor having a substantially planar base defining an opening for overlying an operative site on a patient, and at least one retractor blade slidably mounted to the base. The base is positioned on the patient such that the opening therein overlies the operative site. The operative site is percutaneously accessed, and obstructing tissue is retracted by engaging the tissue with the retractor blade. A surgical instrument is provided which is engageable with the base and operable at the operative site through the opening in the base. A surgical procedure is carried out through the opening in the base with the surgical instrument.
