    • A clamp is provided for attachment to the left atrial appendage (LAA). The clamp can be locked in a closed position so as to remain in place on the LAA. After being attached to the LAA, the clamp can be moved to a desired position such that the LAA can be retracted to a desired extent. The clamp can be held in any desired position, preferably by being connected to a thoracic retractor that is being used to retract the patient's sternum or ribs. A movable member is connected between the clamp and a stationary portion of the retractor. By locking the movable member in a fixed position relative to the stationary portion, the clamp can be maintained in a desired position and the LAA can be retracted as desired.

    • A retractor for use in various types of surgical procedures such as harvesting mammary arteries, coronary bypass surgery, heart valve repairs, mitral valve replacement, partial sternotomies, and other types of surgery includes a toothed crossbar to which a pair of small grips are attached. The first grip is attached to an arm which is removably connected to a first block. The first block is movable along the crossbar, thereby permitting the first grip to be moved toward or away from the second grip. The second grip is attached to an arm which is removably connected to a second block. The second block can be fixed or movably connected to the crossbar. The crossbar includes a hinge that enables the grips to be pivoted relative to each other. Pivoting is accomplished by a pair of vertically extending brackets that are connected to the blocks and which are connected to each other by an adjustable connector. Retractor blades of various types are connected to the adjustable connector or to arm-carried rods by universal clamps. The grips can be removed and replaced with differently configured grips in order to conduct different types of surgical procedures or to conduct surgical procedures on different portions of a patient's body. The retractor according to the invention enables one portion of a patient's body to be both retracted and raised relative to an adjacent portion of the body. Such retraction provides effective access to a surgical site with minimal invasion of the body.

    • A device for occluding a conduit includes first and second members, each with first and second ends. The second ends are connected by a hinge. The first ends interlock to close the device about the conduit. The members are shaped such that they can occlude the conduit without causing the development of damaged or necrotic tissue. If desired, a flexible cover can be provided and the first and second members can be disposed within the cover. If such a cover is used, it will contact the conduit in order to cushion the contact between the members and the conduit, minimize the tendency of the conduit to bleed, and increase traction relative to the conduit. The invention also includes methods for using the device to occlude a conduit.

    • An aorta cross clamp assembly includes a clamp having movable jaws and a removable actuator having movable handles. Initially, the actuator is connected to the clamp with the jaws in an open position. When the actuator handles are closed, the jaws also will be closed. The clamp is provided with a toothed retainer that prevents the jaws from being opened accidently. The actuator can be detached from the clamp and removed from the operative site, leaving the locked clamp in place. When it is desired to remove the clamp, the actuator can be reattached to the clamp and used to disengage the toothed retainer. The actuator then can be used to spread the jaws in order to permit the clamp to be removed. The clamp and the actuator are provided in two forms. In one form, the toothed retainer is engaged and disengaged by movement toward and away from the jaws (“horizontal” movement). In the other form, the toothed retainer is engaged and disengaged by movement generally perpendicular to the jaws (“vertical” movement). Another embodiment employs a clamp having movable jaws and a slender, flexible, actuator that is not intended to be removed during use. The jaws are actuated by axial movement of a screw that is connected to the end of a cable included as part of the actuator. The screw passes through a slotted nut that permits non-rotational axial movement of the screw in one direction to close the jaws, but which requires that the screw be rotated in order to move in the opposite direction to open the jaws.

    • A surgical retractor includes a pair of small grips that are disposed at the ends of arms that can be moved toward or away from each other. The grips engage and separate the sides of an incision that has been made in a patient. A lifting mechanism that can be attached to one of the arms enables one side of the incision to be raised relative to the other side of the incision. The lifting mechanism includes a clamp that can be connected to the retractor and a foot that is connected to the clamp and which is movable relative thereto. The foot is engageable with the patient such that extension of the foot causes a selected grip to be raised. Retraction of the foot permits the selected grip to be lowered. In the preferred embodiment, the foot is disposed at one end of a notched rack; a rotatable pinion is in contact with the notches to extend and contract the rack; and a pivotally movable pawl permits movement of the rack in one direction and prevents movement of the rack in the other direction.

    • An intervertebral spacer suitable for use as a spinal implant includes a rigid, porous body made of a biocompatible material. The body is strong enough to accommodate loads imposed by adjacent vertebrae and is porous enough to facilitate tissue ingrowth and bony fusion. The porosity is provided by a plurality of randomly sized, substantially interconnected voids disposed throughout the body. The spacer can be manufactured by a variety of techniques. In one technique, a plurality of polymer pellets are placed in a mold of a desired shape and heated to melting or near-melting temperature. The pellets are fused together to form a porous, rigid structure. In another technique, tortuous strands of a biologically inert material are intermixed with each other and are sintered into a desired shape in a mold. The spacer also can be made of a sintered mixture of strands and pellets of a biologically inert material. The material for the strands and the pellets typically is titanium or a titanium alloy, although other metals and certain polymers such as PEEK can be used. In another techniques, the spacer is made of void-containing foam metal, void-containing powdered metal, or void-containing ceramic. The spacer also can be made from a solid block of a biocompatible material by drilling or machining a plurality of interconnected openings that extend through the block. The spacer can be provided in a variety of shapes to suit the needs of individual patients.
