摘要： Surgical fastener applying apparatus having an articulated shaft assembly intermediate a proximal actuator assembly and a distal fastener applying assembly. The rotational axis of the articulation is transverse to the proximal-distal axis of the shaft assembly. The apparatus is fully operational at any rotational position of the articulation. A detent mechanism may be included to releasably hold the articulation in any of its rotational positions. A locking mechanism may also be provided for locking the articulation in any of its rotational positions. The shaft assembly may also include a second articulation whose rotational axis is the proximal-distal axis. The apparatus is fully operational at any rotational position of the second articulation, and detent and locking mechanisms may also be associated with the second articulation.

摘要： Surgical instrumentation and methods for performing a bypass procedure in a digestive system incorporate laparoscopic techniques to minimize surgical trauma to the patient. The instrumentation includes an outer guide member dimensioned for insertion and passage through an esophagus of a patient and defining an opening therein extending at least along a portion of the length of the outer guide member, an elongate anvil delivery member at least partially disposed within the opening of the outer guide member and being adapted for longitudinal movement within the outer guide member between an initial position and an actuated position and an anvil operatively engageable with the delivery member. The anvil includes an anvil rod defining a longitudinal axis and an anvil head connected to the anvil rod. The anvil head is at least partially disposed within the opening of the outer guide member when in the initial position of the delivery member and is fully exposed from the distal end of the outer guide member upon movement of the delivery member to the actuated position.

The tensioning mechanism includes a cinch track engageable with one of the pair of sutures.

A buttress material including at least one aperture for receipt of the at least one projection at a first end of jaws and a tab configured to engage the at least one jaw at a second end thereof is also provided.

Preferably the head and the barrel portion define a throughbore for receipt of a drive instrument. A thread on the head is provided to engage threads in the installation tool. A tissue thread is provided on the barrel portion to engage tissue. Distal and proximal surfaces of the tissue thread may be oriented at various angles relative to the barrel portion. There is also disclosed an insertion instrument to insert one or more tacks as well as a method of use. There is further disclosed a model device for use in explaining the operation of the instrument.

摘要： A mechanism is provided for crimping and releasing a suture securing a strip of buttress material to an end effector. The mechanism includes a retention member movably mounted within the end effector. The retention member is movable from a first position crimping the suture to the end effector to a second position releasing the suture from the end effector. The mechanism additionally includes a follower member, engageable with the retention member, and movable in response to actuation of a surgical instrument to move the retention member between the first and second positions. A lock mechanism is also provided to secure the retention member in the first position. The follower member operates to disengage the lock mechanism.
