    • An implantable cardiac defibrillation assembly includes at least one implantable lead having a defibrillation electrode adapted for placement in a chamber of the heart. The lead includes a connector. The assembly further includes an implantable defibrillation device having a pulse generator that provides defibrillation pulses and that is configured to receive the connector to couple the defibrillation electrode to the pulse generator. The device further includes a system that evaluates and conditions the assembly to provide defibrillation therapy to the heart without requiring arrhythmia induction of the heart. The system may condition the device for defibrillation therapy by reforming the defibrillation output capacitor and evaluate defibrillation lead DC resistance, and R wave sensing and detection. In addition, the system may estimate defibrillation thresholds and electrical fields and condition the device by setting the device to provide an output voltage above the estimated threshold. All of the foregoing may be accomplished without inducing ventricular fibrillation.

    • A combined programming wand and PSA for physician programmers. A programmer/PSA wand can be connected to known programmers and PSA application software can be installed on the programmer to provide a system with the functionality of a PSA and a programmer in a single piece of clinical equipment.     • An implantable cardiac stimulation device is described wherein a controller of the cardiac stimulation device controls selected functions of the device based on whether the patient is at rest and further based on whether the patient is prone to vagally-mediated arrhythmias. Functions of the device that may be controlled include, for example, a pacing base rate, an AV/PV delay, and a refractory period as well as overdrive pacing parameters and diagnostic data gathering parameters. In one example, if the patient is not prone to vagally-mediated arrhythmias, the base rate is lowered while the patient is at rest. Also, overdrive pacing parameters are set to be less aggressive. As such, the operation of the cardiac stimulation device is controlled to make it easier for the patient to rest while also reducing power consumption. However, if the patient is prone to vagally-mediated arrhythmias, the base rate is not lowered while the patient is at rest. Overdrive pacing parameters are instead set to be more aggressive, rather than less aggressive. In this manner, the cardiac stimulation device attempts to compensate for any increased risk of arrhythmia that may occur while the patient, who is prone to vagally-mediated arrhythmias, is at rest. Numerous other parameters may be adjusted dependent upon whether the patient is at rest or dependent upon whether the patient is prone to vagally-mediated arrhythmias.

    • An implantable cardiac device detects a progression or regression in heart disease such as congestive heart failure. An activity sensor and a respiration sensor generate raw signals indicative of the patient's activity level and respiration level. Degradation or improvement of the patient's activity and respiration over a predetermined time corresponds to an indication of the progression or regression of the heart disease. A processor coupled to the sensors is programmed to process the raw sensor signals over the predetermined time and stores the processed sensor signals in a memory having a data storage area. A telemetry circuit coupled to the memory is configured to transmit the stored sensor signals to an external monitor for subsequent display. The processor further controls pacing of the heart, adjusts pacing therapy responsive to the process signals, and process the raw respiration signals when the patient is in a number of different active states.

    • Techniques are provided for delivering cardiac pacing therapy to the heart of a patient using an epicardial left ventricular satellite pacing device in conjunction with primary pacemaker having at least a right ventricular pacing lead. In one embodiment described herein, right ventricular pulses generated by the primary pacemaker are detected by the satellite pacer and analyzed to determine the timing pattern employed by the primary pacemaker. The timing pattern is then used to specify the delivery times of epicardial left ventricular pulses so as to be synchronized with right ventricular pulses. In another embodiment described herein, the primary pacemaker modulates the right ventricular pulses to encode timing information, which is then detected and decoded by the satellite pacemaker. In this manner, biventricular pacing therapy, such as cardiac resynchronization therapy, may be conveniently delivered using a non-biventricular pacemaker in combination with an epicardial satellite pacer.
