The reflection surface of the optical element reflects all incident lights to guide the lights to the optical propagation member. A surface where a second electrode is disposed is formed in an end including the reflection surface of the optical element. While the optical element is mounted on the slider, the end surface where the first electrode of the slider is disposed and the surface where the second electrode of the optical element is disposed are continuous.
