摘要： The disclosed technology includes a system for applying pulsed field ablation to treat atrial fibrillation. The system includes a catheter and a processor. The catheter can include a tip electrode controlled to emit either a pulsed electric field or a radiofrequency signal to biological tissue and ablate one or more locations of biological tissue of the pulmonary vein. The processor is configured to measure a contact force experienced by the tip electrode against biological tissue during pulsed electric field ablation and determine a pulsed field ablation index as a function of the measured contact force and number of pulsed electric field applications for each location of the one or more locations so that it can be determined whether a lesion is sufficiently durable.

摘要： An electro-surgical pencil 1 is provided. The pencil comprises a housing 2 forming an axially extending main suction passage from a proximal opening to a distal opening. The housing comprises (i) a main body 11 forming the axially extending main suction passage and the distal opening, and (ii) a switch 6, 7 on the main body. The pencil further comprises a tube 3 with an internal conduit extending between a distal extension opening and a proximal extension opening and forming an extension of the main suction passage to the proximal extension opening, the tube being configured for telescoping movement through the proximal opening. The pencil still further comprises an electrode 5 for cutting and/or coagulating tissue, the electrode being attached to the tube and extending axially from the proximal extension opening, wherein the switch is configured to cause an electrosurgical radiofrequency, RF, signal to be supplied to the electrode, and a suction tip 9 attached to the tube and forming a further extension of the suction passage. While the electrode remains in a fixed position relative to the tube, the suction tip is configured for telescoping movement along an outer surface of the tube to cover a larger or lesser portion of the electrode, wherein the suction tip is receivable in a recess in the housing, such that: (i) a distance along an axial direction between the tube and the switch is smaller than a largest dimension of the housing in a cross section perpendicular to the axial direction, or (ii) the tube and optionally a portion of the suction tip is configured to slide below the switch such that the tube and the switch overlap each other in a direction perpendicular to the axial direction of the pencil.
