摘要： An apparatus for introducing a first elongate medical device (10) and short wire guide (11) that are coupled together into a work site and remotely disconnecting them within the work site such that a secondary device comprising a catheter member can be introduced over the wire guide to the work site, and/or a second wire guide can be introduced to the work site via a passageway of the primary access device. A separating member may be provided to remotely separate the wire guide from the elongate medical device. A system of indicia, such as radiopaque or viewable markers, permits the operator to monitor the relative alignment of the devices within the work site to determine when uncoupling has occurred.

摘要： The present invention provides a balloon cuff (20) comprising an attachment structure adapted to be disposed around an exterior surface of an endoscope, and a balloon (30) coupled to the attachment structure. An inflation means (40) is in fluid communication with the balloon. The balloon has a deflated state suitable for insertion into a body passage and an inflated state suitable for urging stone fragments along the body passage. Optionally, an intraductal shock wave lithotripsy procedure may be performed through the working lumen of the first endoscope (50) while the balloon cuff is disposed, either inflated or deflated, about the exterior surface of the first endoscope. The first endoscope and balloon cuff may be used alone to remove stone fragments, or alternatively, may be used in conjunction with a second endoscope, such as a duodenoscope, having a working lumen sized to received the first endoscope having the balloon cuff disposed thereon.

摘要： A steerable overtube is provided having an elongate shaft comprising an oversized accessory channel that is configured for the introduction and advancement of elongate medical devices having relatively large cross-sections. The distal end of the overtube shaft is bendable or deflectable in at least one direction, and is preferably includes a shape locking mechanism for temporarily maintaining the shape of the distal end of the overtube shaft. An oversized accessory channel is provided. The overtube may also include a fixation mechanism for securing the proximal end and/or distal end of the overtube against movement relative to the patient. A fiber optic elongate medical device is provided for use with the steerable overtube of the present invention.

摘要： A system is provided for preventing the accumulation of cumulative matter from clogging a stent, such as a plastic tubular drainage stent for use in the biliary system. The system includes a stent having one or more magnetically reactive objects moveably disposed therein and an actuation device, such as a glove, having one or more magnets thereon. When the actuation device is waived near the one or more magnetically reactive objects of the stent, it actuates them by manually manipulating the one or more magnetically reactive objects of the stent by attracting or repelling them such that they contact the stent walls dislodging cumulative matter attached thereto. As a result of the system, the stent is able to remain resident within the patient for an extended period of time before becoming clogged and needing to be replaced.

摘要： A suturing device for apposition of tissues is disclosed. The device comprises an introducer having a first port, a second port, and a suture port formed therethrough. First and second needle assemblies are disposed through the first and second ports, respectively. First and second ferrules are removably disposed in the first and second ports, respectively, at a distal end of the introducer. The first and second ferrules are configured to receive the first and second assemblies, respectively, for tissue apposition. A fastener for attaching suture wires is disposed in the suture port at the distal end of the introducer. First and second suture wires are attached to the first and second ferrules, respectively, and extend through the fastener for apposition of tissues.
