摘要： Methods and apparatus for delivery of substances or apparatus to target sites located outside blood vessels within the body of a human or animal patient. A vessel wall penetrating catheter is inserted into the vasculature, positioned and oriented within a blood vessel near the target extravascular site and a penetrator is advanced from the catheter so as to penetrate outwardly through the wall of the blood vessel in the direction of the target site. Thereafter, a delivery catheter is passed through a lumen of the penetrator to the target site. A desired substance or apparatus is then delivered to or obtained from the target site. In some applications, the penetrator may be retracted into the vessel wall penetrating catheter and the vessel wall penetrating catheter may be removed, leaving the delivery catheter in place for chronic or continuous delivery of substance(s) to and/or obtaining of information or samples from the target site. Alternatively, a delivery catheter having an occlusion member or balloon may be advanced into a vein or venule and the occlusion member or balloon may be used to occlude the lumen of the vein or venule during and after injection of a substance through the catheter, such that the substance will not be carried away by normal venous blood flow and will remain in the vein or venule for a sufficient period of time to have its intended effect (e.g. to enter adjacent tissues through capillary beds drained by that vein or venule).

The flexible occluder member (14) may be of generally tublular form having a closed end (18) and an open end (16). The open end (16) is attached to the frame (12) and the closed end serves to substantially block the flow of blood through the lumen of the anatomical conduit. In some embodiments a small or self-sealing opening (21) is formed in the closed end (18) of the flexible occluder member (14) such that a guidewire, catheter (40) or other device may pass through such opening during delivery of the device and/or at some later time following implantation of the device.

摘要： Apparatus (10) for creating channels between adjacent blood vessels include a cutting element (22) fixed on a catheter (12) for creating a channel in a wall of a blood vessel. A clip (34) engages tissue thereunder for maintaining the cutting element (22) in contact with tissue being cut. Orientation markers (32, 33) may facilitate rotationally orienting the cutting element (22). The catheter (12) is advanced into a blood vessel, and a distal portion (24) of the catheter (12) is directed through a wall of the vessel towards an adjacent vessel until the cutting element (22) is in contact with tissue therebetween. The catheter (12) is directed axially along the vessel, thereby creating a channel in the tissue using the cutting element (22). 摘要： A trans-vascular system (10) for delivering a drug to a tissue region from a blood vessel, such as a coronary vein, includes a catheter (12) having a distal portion (26) with puncturing (14), orientation (16), drug delivery (62), and imaging elements (18). The puncturing element (14) is deployed for penetrating the vessel wall to access the tissue region. The orientation element (16), e.g., a 'cage' including a plurality of struts (38, 40), and/or a radiopaque marker has a predetermined relationship with the location of the orientation element (16) with respect to the tissue region to orient the puncturing element (14). The catheter (12) is percutaneously introducing into the vessel, the puncturing element (14) is oriented towards the tissue region, the puncturing element (14) is deployed to access the tissue region, and the drug is delivered to the tissue region. An ablation device (230) may also be deployed to create a cavity in the tissue region for receiving the drug therein, or an indwelling catheter (214) may be advanced into, and left in the tissue region. An implanted reservoir device (350) is also disclosed, including an enclosed membrane (354) on an expandable frame (352) that defines a reservoir, and includes a porous region. The reservoir device may be deployed, expanded within a blood vessel, and may be filled in situ or pre-filled with a drug that passes through the porous region. Alternatively, a pair of expandable endovascular blockers may be used to isolate a section of a blood vessel which may be filled with a drug that may be absorbed by the surrounding tissue.
