    • An improved guide wire for use with cardiovascular catheters, particularly angioplasty or balloon dilatation catheters. The guide wire has a solid core wire which makes up the proximal end of the guide wire. The core wire is tapered toward the distal end to increase flexibility. The tapered distal end of the core wire is surrounded by a coil spring, which may be made completely or partially from a highly radiopaque material. A tapered transition device is attached to the coil spring at a selected distance from the distal end of the guide wire. The transition device is roughly bullet-shaped with a tapered distal edge which provides a smooth transition to aid the dilatation catheter in crossing a stenosis. The tapered transition device also serves to predilate very tight stenoses, making it easier to cross the lesion with an angioplasty balloon or other therapeutic device. The transition device can be made of metal or plastic with a radiopaque filler for easier visualization by x-ray or fluoroscopy. A removably attachable handle for maneuvering the guide wire and a method of use for the improved guide wire are also disclosed. An angioplasty catheter with a reinforced proximal shaft, which allows easier one-handed operation of the catheter and guide wire system, is also presented.

    • Described herein is a catheter method and apparatus for the guided intraluminal bypass of a coronary occlusion. A method is described employing an intraluminally operable catheter shaft including an optionally steerable working element, combined with an extraluminally operable locator including an imaging device, to selectively direct the working element to create a fluid path from the artery proximal to the occlusion into the vein and further from the vein back into the artery distal to the occlusion, to perfuse myocardium distal to the occlusion. Methods are described for creating a fluid path from the artery proximal to the occlusion into an adjacent vein and thereafter either stent grafting the fluid path to the vein, directing arterial blood distally within the vein, or stenting the fluid path and occluding the vein proximal thereto, in either case arterializing the venous system distal to the stent or stent graft. A catheter shaft is described for creating and perfusing the fluid path. Conical, guide wire and tissue-penetrating working elements are described. A scanning acoustic transducer and flexible imaging tube, and suction devices for anchoring are described for the locator. A signal-emitting working element and signal-detecting locator cooperating therewith are described.

    • An apparatus is described for treating arterial occlusions combining an intraluminally operable catheter having an occlusion-crossing working element, with an extraluminally operable locator for imaging the progress of the working element through the occlusion. Suction coupling means are described for removably anchoring the locator to a surface proximate the heart. A method for treating arterial occlusions is described in which the distal end of the catheter shaft exits a lumen of the locator, penetrates the arterial wall distal to the occlusion, crosses the occlusion retrograde and passes into a separately introduced standard intravascular catheter, whereupon the distal end of the shaft may be manipulated from the proximal end of the standard catheter. The proximal end of the shaft is then released from external attachments and drawn into the artery to perform treatment or guiding functions. Spatial interrelationships are observed real-time via the locator. Conical, abrasive, blunt-dissecting, sharp-pointed and guide-wire type working elements, steerable and non-steering, are described. Acoustic transducers and a flexible imaging tube are described for the locator. A signal-emitting working element and cooperating signal-receiving locator are described.
