    • A probe for use with an endoscopic suction-irrigation instrument is disclosed which permits a second probe to be inserted therethrough. The suction-irrigation instrument has a cannula, a fluid chamber having at least one port for irrigation and suction and a proximal opening, and a first seal in the proximal end of the fluid chamber. The fluid chamber has a distal end communicating with the cannula. The probe has a proximal handle portion, a middle portion, and a hollow distal portion. The handle portion has an axial passageway and a second seal therein. The middle portion extends from the handle portion through the first seal at the end of the fluid chamber and through the fluid chamber. The middle portion has an outer diameter shaped to contact the first seal such that the proximal end of the fluid chamber is sealed. The middle portion is also shaped to be smaller than the inner diameter of the fluid chamber, and the middle portion is provided with a second axial internal passageway extending therethrough. The distal portion extends at least partially through the cannula and has an outer diameter smaller than the inner diameter of the cannula. A second probe may be inserted through the second seal of the first probe, through the axial passageway in the handle portion, through the internal passageway in the middle portion, and out through the distal portion and the cannula.

    • A cautery probe is described for use with an endoscopic instrument where the endoscopic instrument has an electrical contact located therein. The probe includes an insulated handle which extends out of the instrument, a conductive probe element which is mounted in the insulated handle and extends through the distal end of the endoscopic instrument, and an insulating cover. The conductive probe element has a distal cautery surface, and an electrical contact surface at a location between the proximal end of the conductive probe and the distal cautery surface, with the electrical contact surface contacting the electrical contact in the endoscopic instrument when said cautery probe is inserted in the endoscopic instrument. The insulating cover substantially covers the conductive probe element from the insulated handle up to but not at the electrical contact surface, and substantially covers the probe element from a point beyond the electrical contact surface, up to but not at the distal cautery surface. Preferably, the cannula of the endoscopic instrument is used as the electrical contact, and the electrical contact surface of the probe element is a bump which contacts the cannula.

    • An electrocautery suction/irrigation endoscopic instrument having an electrocautery safety override is provided. The instrument preferably includes a cannula, an irrigation conduit in fluid communication with the cannula, an irrigation valve for regulating fluid flow in the irrigation conduit, an actuator coupled to the valve, an electrocautery probe coupled to a source of current, and an electrical switch coupled to the source of current and to either the irrigation valve or to the actuator. When the actuator is activated to permit fluid flow through the irrigation conduit, the switch is switched and prevents the flow of cautery current from the current source to the electrocautery probe. Preferably, the switch opens a DC circuit which is used to signal the current source to provide an high voltage AC electrocautery current. Alternatively, the switch may be located in the high voltage AC circuit. Both foot and thumb activated cut/coag switches are disclosed for use with the instrument.

    • A vacuum powered pump includes a spring biased piston or diaphragm in a vacuum chamber coupled to a source of vacuum. When vacuum is applied to the vacuum chamber, the piston or diaphragm is moved in a first direction against the spring. A delayed action valve is coupled to the vacuum chamber to allow ambient air to enter the chamber only after the piston or diaphragm has been moved a substantial distance in the first direction. When the delayed action valve opens, air enters the vacuum chamber and the force of the entering air together with the bias of the spring propels the piston or diaphragm in a second direction opposite the first direction. The delayed action valve automatically closes and the process repeats so long as a vacuum source is coupled to the vacuum chamber. The reciprocal movement of the piston or diaphragm effects a pulsatile pumping of fluid through a fluid chamber having at least one check valve.

    • An apparatus for counting the number of times a medical instrument has been sterilized includes an indicator having sequential indicia and a heat responsive member for indicating a next one of the indicia. Mechanical, fluid mechanical and electronic versions of the invention are disclosed. In the mechanical version, the indicator can be a ratchet member having teeth, a pawl and a display surface containing indicia. A heat responsive element such as a bimetallic member engages the teeth of the ratchet and advances the ratchet each time the apparatus is subjected to the heat of sterilization. A housing with a window masks the display to indicate one of the indicia as advanced by the heat responsive element. Preferred embodiments of the mechanical version include a ring-like ratchet member with interior teeth and an exterior display surface. The number of teeth is preferably one or more less than the number of indicia so that upon advancing the ratchet member to the last indicia, it can be advanced no further. The bimetallic element is mounted inside the ring and the housing covers the outer surface of the ring. The housing may be formed from the ferrule of an endoscopic surgical instrument or may be formed as part of the handle of such an instrument. A removable locking pin is provided to prevent movement of the ratchet member during shipping and the locking pin may be coupled to packaging so that it is automatically removed when the surgical instrument is removed from its packaging.
