摘要： An instrument and method are provided for sealing and joining or hemostatically dividing tissue, which is particularly suitable for laparoscopic and endoscopic surgery. The instrument makes use of the controlled application of a combination of heat and pressure to seal adjacent tissues, to join adjacent tissues, or to anastomose tissues, whereby tissue is heated for an optimal time and at an optimal temperature under optimal pressure to maximize tissue seal strength while minimizing collateral tissue damage. The instrument of the present invention is lightweight and therefore portable, and is particularly useful in field conditions where a source of external power may not be readily available.

摘要： An expansible laparoscopic tunneling apparatus for creating an anatomic working space. The apparatus provides laparoscopic observation both during tunneling operations, when the apparatus is advanced bluntly to a desired location within the body of a patient, and during subsequent tissue dissection when a balloon associated with the apparatus is inflated to create the working space. In a preferred embodiment, the apparatus comprises a hollow tunneling member and an inflatable balloon disposed at the distal end of the tunneling member to cooperatively form a blunt tipped obturator. The tunneling member has a bore sized to accept a conventional laparoscope, and a blunt distal end to facilitate tunneling. A trocar can be inserted into the incision to provide access back to the previously created space where the surgical procedure is to be performed. The apparatus is prepared for use by inserting the tunneling member into the interior of the balloon, forming the balloon into a generally cylindrical roll about the tunneling member, and preferably, through the use of an integral balloon cover, securing the balloon roll to the tunneling member wherein the combination of the tunneling member and rolled balloon forms a semi-rigid blunt tipped obturator. A horseshoe shaped balloon for dissecting around obstructions within the body is also disclosed.

摘要： In a method of making a surgical valve, a sealing body is attached inside one end of a braided sleeve, the other end of which is threaded through the bore of a toroidal body. The braided sleeve, toroidal body, and sealing body are inserted into the bore of a substantially cylindrical valve body. The sealing body generally has an axial passage extending through it which is axially aligned with the toroidal body. The braided sleeve is also typically coaxial with the axial passage and extends from the sealing body, and a device that selectively changes the relative axial positions of the sealing body and the extended part of the braided sleeve elongates the braided sleeve. This causes the braided sleeve to contract radially, which compresses the axial passage of the sealing body radially.

摘要： An adjustable surgical valve has a sealing body with an axial passage extending through it, a toroidal body axially aligned with the sealing body, and a device that selectively changes the relative axial positions of the sealing body and the toroidal body. The sealing body and the toroidal body have mating surfaces which radially compresses the axial passage of the sealing body when the relative axial positions of the sealing body and the toroidal body are changed. This causes the axial passage to seal with an instrument inserted into it, or the seal with itself if no instrument is inserted. Alternatively, or additionally, the sealing body may include a braided sleeve coaxial with the axial passage and extending from the sealing body, and a device that selectively changes the relative axial positions of the sealing body and the extended part of the braided sleeve to elongate the braided sleeve. This causes the braided sleeve to contract radially, which compresses the axial passage of the sealing body radially. The sealing body is made of an elastomeric foam or an encapsulated gel.
