摘要： A jaw assembly is provided with a pair of jaws which are closed with a flexible, tension member that is pulled by the operator of the instrument. In a preferred embodiment, one of the jaws is pivotally mounted in a slot to the other jaw. A cord, having a loop configuration, is attached to the pivoting jaw and can be pulled to pivot the jaw closed. A second cord can be provided to initially hold the proximal end of the pivoting jaw away from the other jaw until the distal ends of the jaws are closer together. In another embodiment, jaws can be closed in a parallel orientation by rotating two pulleys with threaded shafts engaged with the jaws.

The first and second elongate clamping portions (2, 4) respectively include ends coupled together with respective resilient urging members (6, 8) configured to urge at least one of the first and second elongate clamping portions (2, 4) toward the other of the first and second elongate clamping portions (2, 4) from an open position into a clamping position to occlude the hollow anatomical structure (40). The clamp (10) is shaped annularly such that it may surround the hollow anatomical structure (40) in the open position and reorient into a flattened shape in the clamping position to occlude the hollow interior of the hollow anatomical structure (40). Apparatus of the invention further include clamp delivery and actuation devices (80, 200) adapted to deploy the clamp (10).

摘要： Apparatus and methods for repairing damaged tendons or ligaments. Various repair apparatus include an elongate tensile member 16 and a pair of anchor assemblies 10 connected for movement along the tensile member 16 on either side of a repair side, such as a tear or laceration. The anchor assemblies or structures 10 may take many forms, and may include barbed, helical, and crimp-type anchors. In the preferred embodiments, at least one anchor structure 10 is movable along the elongate tensile member 116 to assist with ajusting a tendon segment 100a, 100b to an appropriate repair position and the anchor structure 10 or structures 10 are then lockable onto the elongate tensile member 116 to assist with affixing the tendon 100 at the repair position. Tendon and/or ligament-to-bone repair apparatus and methods employ similar concepts.

摘要： An open thoracotomy surgical technique for aortic valve replacement is disclosed. A surgical incision (200) is made in one of several locations such as a midline sternotomy incision, a small anterior right or left thoracotomy, a mini-thoracotomy, a posterior thoracotomy, a suprasternal supra-clavicle approach, or through port sites (mini incisions) over the chest wall. The pericardium is then opened. The faulty natural aortic valve, next to the left ventricle (2), is excised and removed. A fastener driving tool with a cuff (19) attached is inserted through the incision (200). The knob (36) is rotated while holding handle (35) to deploy the fasteners through the cuff (19) to secure it. Finally, a prosthesis valve body (20) is attached to the cuff (19) before closing the incision (200).

摘要： Query electrodes are provided for monitoring the electrical impedance of tissue as it is treated with electrosurgical energy. In one embodiment, based on a predicted model of tissue impedance and a number of initial impedance readings, the impedance at which tissue treatment is completed is predicted. More particularly, a minimum impedance level is measured and a function of the minimum impedance is used to determine impedance at which coagulation is completed. A control device is provided for bringing the output of the generator within an optimum range based on a system load curve. In one embodiment the impedance monitoring device is used in conjunction with a bipolar electrosurgical instrument. Preferably, the instrument comprises electricall y opposite therapeutic electrodes, each located on one or more tissue engaging surfaces for engaging tissue to be treated, and electrically opposite query electrodes, located on one or more tissue engaging surfaces. The therapeutic electrodes provide therapeutic energy to the tissue while the query electrodes provide a lower voltage electrical sensing energy for the purpose of measuring tissue impedance at various stages, e.g. before, during, or after tissue treatment.

摘要： A clamping and fluid delivery device (50) for occluding a vessel (12) during a surgical procedure. Generally, the device (50) includes an internal core portion (52) having a distal end (52a) with a sealing surface and opposite side surfaces comprising sealing surfaces. The core portion (52) is inserted transversely into the vessel (12). Opposed, external clamping arms (62, 64) move together outside the vessel (12) and clamp the vessel (12) against the core portion (52). Padding (124) the sealing surfaces on opposite sides of the core portion (52), as well as on the distal end (52a) engage the internal walls (12a) of the vessel (12) and are opposed by padded clamping surfaces (120, 122) of the arms (62, 64). The core portion moves distally simultaneously with the clamping action of the arms to provide a distal seal. A bypass cannula (58) and cardioplegia cannula (60) fluidly couple the core portion (52) to deliver blood and cardioplegia fluid to opposite sides of the core portion (52).
