    • There are disclosed various embodiments of a balloon dissector and balloon tip cannula assembly which are provided to facilitate forming an anatomical space within the body, such as, for example, an anatomical space in the abdominal cavity or extraperitoneal space for facilitating hernia repair surgeries. The balloon dissector and balloon tip cannula assembly generally includes a balloon tip cannula assembly for anchoring the device to the abdominal wall and a balloon dissector assembly having a dissection balloon at a distal end for separating apart tissue layers and forming an anatomical space. Various structures are provided to connect the balloon dissector assembly to the balloon tip cannula assembly. Obturators associated with the balloon dissector assembly and the balloon tip cannula assembly may also be provided. The balloon dissector and balloon tip cannula assembly may also include a scope support for insertion through the balloon dissector assembly and dissection balloon and for receipt of an endoscope to view the procedure from within the balloon. Various combinations of valve ports and sub-assemblies are disclosed for providing inflation fluid to the balloon tip cannula assembly, insufflation fluid for the abdominal cavity and inflation fluid for the dissection balloon.

    • The present disclosure is directed to a surgical stapling device (10) for performing circular anastomoses. The surgical stapling device includes a handle portion (12), an elongated body portion (14) and a head portion (16) including an anvil assembly (30) and a shell assembly (31). The handle portion (12) includes a rotatable approximation knob (22) for approximating the anvil and shell assemblies (30,31) and a firing trigger (20) for actuating a firing mechanism for ejecting staples positioned within the shell assembly (31). The firing trigger (20) forms one link of a two bar linkage provided to actuate the firing mechanism. The anvil assembly (30) includes a tiltable anvil which will tilt automatically after firing of the device and unapproximating the anvil and shell assemblies (30,31). The head portion (16) also includes a retractable trocar assembly (240) which is slidably positioned within an anvil retainer (38) and is automatically advanced and retracted upon attachment and detachment of the anvil assembly (30) onto the anvil retainer (38). A lockout tube (270) is provided and is positioned about the anvil retainer (38) for releasably engaging an anvil assembly (30). The lockout tube (270) prevents inadvertent detachment of the anvil assembly (30) from the anvil retainer (38). The surgical stapling device (10) also includes a firing lockout assembly which prevents actuation of the firing trigger (20) until an anvil has been attached to the device and the anvil (129) has been approximated. A tactile indication mechanism is also provided for notifying a surgeon that the device has been fired and for notifying a surgeon that the anvil head has been unapproximated a distance sufficient to permit the anvil head to tilt.

    • A surgical clip applicator comprising a housing, a pair of handles pivotally connected to opposite sides of said housing, and a jaw blade assembly fixedly connected to the housing. The jaw blade assembly includes a pair of jaws for receiving and deforming a clip therebetween and a clip carrier for supplying a series of clips to said jaws. A channel assembly is slidably mounted in the housing and envelops said jaw blade assembly. A forming cam is connected between and to said handles and said channel assembly for sliding said channel assembly in a distal direction in response to closing of said handles together to move said jaws towards each other. A feed cam bar slidably mounted in said channel assembly in overlying relation to said clip carrier, said bar having a nose at its distal end. A pusher bar slidably mounted in said channel assembly for advancing clips along said clip carrier. A first spring member connected between said cam bar and said forming cam for sliding said cam bar in a proximal direction in response to closing of said handles together and further biasing said channel assembly in a proximal direction. Second spring member biasing said pusher bar in a distal direction to push a foremost clip in said clip carrier between said jaws in response to opening of said handles from each other. A clip retainer for preventing movement of said spring biased pusher bar is also provided.
