摘要： PROBLEM TO BE SOLVED: To provide an endoluminal prosthesis with a steerable branch.SOLUTION: An endoluminal prosthesis includes a tubular main graft body including a sidewall and proximal and distal ends. A first stent is positioned near the proximal end of the main graft body. A second stent is positioned to be adjacent to distal side of the first stent. A hole provided in the sidewall is positioned longitudinally between a peak of the first stent and a valley of the second stent. A tubular branch is disposed in the hole. The branch includes first and second end openings. The branch is flexibly orientable between a retrograde configuration, in which the first end opening is oriented toward the distal end and the second end opening is oriented toward the proximal end, and an antegrade configuration in which the first end opening is oriented toward the proximal end and the second end opening is oriented toward the distal end.

摘要： PROBLEM TO BE SOLVED: To improve a system for inserting an endoluminal prosthesis like a stent graft.SOLUTION: An insertion system may include the stent graft, a locking member and one or more diameter reduction member(s). The stent graft may include a tubular graft including lateral parts that are each disposed to be faced, connected using a tangent line and extended in the first and second longitudinal directions. When the locking member is in a locking position, the locking member restricts a surface of the graft with respect to a cannula. The first diameter reduction member is slidably connected to a first portion of the graft that is arranged approximate the tangent line, and is slidably connected to a second portion of the graft that is circumferentially separate from the tangent line. When the first diameter reduction member is in a restricted position, the second portion of the graft is drawn toward the first portion of the graft, and a proximal portion has a diameter reduction structure with at least two lobes.

The wire extends distally from a first wire end through an axial lumen of the delivery catheter and the prosthesis, and through the fenestration in the graft. The wire extends proximally through a lumen of the prosthesis and through an axial lumen of the delivery catheter towards a second wire end. Additional devices, systems, and methods are also disclosed.

In one embodiment, the prosthesis includes the fenestration capable of pivoting for conforming to a dynamic shape of an aortic branch. In another embodiment, the fenestration capable of pivoting include: a first circumference; a band made of a flexible material that is attached to and surrounds the first circumference; and a second circumference that is attached to and surrounds the band made of the flexible material. The first circumference, the band made of the flexible material and the second circumference are geometrically-shaped. In one embodiment, the prosthesis includes at least three fenestrations capable of pivoting.

Rotation of the cannula subsequently causes the portion of the stent to disengage from the coiled member. A protective cage may encircle the coiled member.
