摘要： A single, multichannel catheter for extracorporeal circulation of blood to a patient undergoing cardiac treatments or surgery. The catheter has three independent channels and an expandable balloon. The first channel is the largest and delivers blood to a patient to maintain the patient's metabolism and perfusion throughout the treatment or surgery. A second, smaller channel is integrated into the wall of the first channel and delivers a biologically active fluid (e.g., for cardioplegia) to the heart and/or venting the left heart. A third, smaller channel is integrated into the wall of the first channel, and delivers an expansion fluid to the balloon to occlude the flow of blood to the heart. Preferably, the first channel accounts for at least about 70% of the total channel volume. The multichannel catheter is best prepared using an extrusion molding technique.

摘要： An adjustable surgical retractor and its use for improving a surgeon's ability to perform closed-chest video-assisted exploratory, diagnostic or surgical procedures on a patient. The surgical retractor has opposable blades which can be inserted into a surgical incision in a patient undergoing a surgical procedure then spread apart to form an elongated access opening through which a instrument may be inserted to perform exploratory, diagnostic or surgical procedures. The blades used in the surgical retractor may be flexible or rigid and are attachable to the retractor. The blades are of a width, depth and thickness to provide an access to an internal cavity or subcutaneous region to allow greater degrees of freedom to the surgeon in inserting instruments into the access opening. The use of the surgical retractor forms a substantially ovoid channel, through which a medical instrument can be inserted to perform surgical or other operations.

摘要： This invention is a single, multichannel catheter useful for extracorporeal circulation of blood to a patient undergoing cardiovascular treatments or surgery. The catheter has three independent channels and an expandable balloon at one end of the catheter. The first channel (34) is the largest and is of a size that allows for delivery of blood to a patient in an amount sufficient to maintain the patient's metabolism and perfusion throughout the treatment or surgery. A second channel (36), smaller than the first, is integrated into the wall of the first channel, and is suitable for delivering a biologically active fluid (e.g., for cardioplegia) to the heart and/or venting the left heart. A third channel (38), also smaller than the first, is integrated into the wall of the first channel, and suitable for delivering a fluid to the balloon for its expansion when positioned in the ascending aorta to occlude the flow of blood to the heart. It is important that the first channel accounts for at least about 70% of the total channel volume. The catheter provides an improved means of performing cardiovascular surgery on a patient using a cardiopulmonary machine for extracorporeal circulation of blood. The catheter is particularly useful for cardiac surgery. The multichannel catheter is best prepared using an extrusion molding technique.

摘要： A surgical retractor is configured to provide surgical access in thoracic surgery, for example through an intercostal incision. The surgical retractor includes at least one inflated or inflatable member to cushion the ribs and surrounding tissue from injury or trauma. In one embodiment, the inflatable member is configured as an inflatable ring with a concave outer surface for engaging and spreading apart two adjacent ribs when the ring is inflated. In another embodiment, the inflatable ring can be combined with a substantially rigid inner ring for supporting and rigidifying the retractor. In yet another embodiment, a plurality of substantially rigid retractor blades is mounted on a spreading track or the like. One or more inflatable members or an inflatable ring are used to cushion the tissue from the rigid retractor blades.

摘要： Devices and methods provide for ablation of cardiac tissue for treating cardiac arrhythmias such as atrial fibrillation. Although the devices and methods are often be used to ablate epicardial tissue in the vicinity of at least one pulmonary vein, various embodiments may be used to ablate other cardiac tissues in other locations on a heart. Devices generally include at least one tissue contacting member for contacting epicardial tissue and securing the ablation device to the epicardial tissue, and at least one ablation member for ablating the tissue. Various embodiments include features, such as suction apertures, which enable the device to attach to the epicardial surface with sufficient strength to allow the tissue to be stabilized via the device. For example, some embodiments may be used to stabilize a beating heart to enable a beating heart ablation procedure. Many of the devices may be introduced into a patient via minimally invasive introducer devices and the like. Although devices and methods of the invention may be used to ablate epicardial tissue to treat atrial fibrillation, they may also be used in veterinary or research contexts, to treat various heart conditions other than atrial fibrillation and/or to ablate cardiac tissue other than the epicardium.

An inflatable balloon is mounted on an outside surface of the sheath. The sheath may also include radiopaque markers, a sterility strip, one or more attachment clips, adhesive attachment, an endcap, and/or an inflation lumen formed as a tube that extends beyond the proximal end of the sheath. The balloons of the sheath may have regular or irregular profiles and shapes, including but not limited to: a bowtie shape, a donut shape, helix, spiral, alternating on opposing sides. The balloon may be simultaneously inflated through a common lumen or they may separately inflatable through separate lumens.
