A second plate includes at least one polyaxial element for connection with a fixation pad and a connection feature for non-rotatable connection with the locking mechanism. The locking mechanism allows translation and polyaxial adjustment of the first plate relative to the second plate and locks the first and second plates via a taper lock. The fixation pad includes a deflectable spacer configured to prevent premature locking of the pad. Instrumentation for implantation, compression and locking of the bone plate assembly, and methods for implantation of the bone plate assembly between two bone structures are disclosed.

    • A line lock (2210) includes a body (2212) at least partially bounding a primary passageway (2222), a secondary passageway (2224), and a working passageway (2228). The passageways receive two locking portions (2256) of a suture (2250) such that the locking portions (2256) can only be moved through the line lock (2210) along one direction. The passageways may be symmetrical, and may enable two ends of the suture (2250) to be drawn in substantially opposite directions to advance the body along the suture (2250). The body (2212) may include grooves (2236) that receive the suture (2250) to enhance locking, and may have a generally circular profile. The suture (2250) may be inserted through the passageways through use of a cartridge (1410) designed to contain the line lock (2210). The suture (2250) may be attached to the line lock (2210) prior to surgical use, either by securing one end of the suture (2250) to the line lock (2210), or by retaining a loop of the suture (2250) with one of the passageways.

    • A system for attaching soft tissue to bone includes an anchor, a suture, and a tissue retainer. The anchor may have a threaded tip that engages the bone, and a suture retention portion with passageways arranged such that each of first and second anchor portions of the suture can be drawn through the passageways along only a single direction. The tissue retainer has passageways through which the suture can freely move in either direction. Thus, the anchor and the tissue retainer may be attached to bone and tissue, respectively, and the suture may be drawn to substantially irreversibly draw the bone and tissue together. In alternative embodiments, an anchor may receive only one portion of suture and/or one suture end may be affixed to the anchor. The anchor may alternatively permit free motion of the suture, while the tissue retainer permits passage of the suture along only one direction.
