摘要： PROBLEM TO BE SOLVED: To provide an endoscopic blood vessel harvesting system which enables simple and convenient operation in a limited space and is suitable for harvesting various target blood vessels with respect to a system, components, and to provide a method for endoscopically harvesting blood vessels. SOLUTION: In an endoscopic blood vessel dissector 170 having a wiper blade or a finger 172 to make a field of view of a distal end dissection tip 174 with respect to tissues clear, the wiper blade 172 rotates about an axis 176 at a beginning position of a dissection tip 174 or an end position of a cannula 178. An activation rod 180 controlled by a thumb lever 182 is connected to the wiper blade 172, and a user moves to make the blade engage an outer surface of the dissection tip 174 or to detach the blade alternately. The tip 174 rotates about the axis of the cannula 178 when a knob 184 of a proximal end part of the device is rotated. 摘要： Apparatus and surgical methods establish temporary suction attachment to a target site on the surface of a beating heart for analyzing electrical signals or hemodynamic responses to applied signals at the target sites for enhancing the accuracy of placement of cardiac electrodes at selected sites and for enhancing accurate placement of a surgical instrument maintained in alignment with the suction attachment. A suction port on the distal end of a supporting cannula carries surface-contacting electrodes and provides suction attachment to facilitate temporary positioning of the electrodes in contact with tissue at the target site, and a clamping and release mechanism to facilitate anchoring a cardiac electrode on the moving surface of a beating heart at a selected site. Analyses of sensed signals or responses to applied signals at target sites promote epicardial mapping of a patient's heart for determining optimum sites at which to attach cardiac electrodes.

To install an embodiment of the element, a carriage (11) holds the element and shields the tines (21) the vessel's open end is passed through the shielded element and fitted over an anvil (25); the tines are exposed, and the carriage is actuated to fire the element against the anvil so that the tines pierce the vessel and curl against the anvil (everting the tissue around the vessel's open end). The anvil preferably forms the vessel end portion into a hooded shape suitable for creating an anastogomotic junction.

The device (20) may be percutaneously inserted into a patient to perform several possible procedures. 摘要： An apparatus and method for separating tissue layers and providing a working space for performing a medical procedure. A first balloon is inserted between the tissue layers and inflated to dissect the tissue layers. A second balloon is also positioned between the tissue layers and inflated to retract the tissue layers. The first balloon is then deflated and punctured with a sharp instrument to create an opening in the first balloon. The second balloon is positioned to impede the escape of insufflation fluid from the working space and the working space is then insufflated. An instrument for performing the medical procedure may then be introduced into the working space through the opening in the first balloon.

The rods are designed to be introduced into the abdominal cavity through a laparoscopic puncture opening and to be extended within the abdominal cavity to provide a lifting area. In the preferred embodiment, a pair of retraction rods (212, 220) disposed with the bores and substantially parallel to each other, are inserted into the abdominal cavity through an incision in the abdominal wall. The retraction rods (212, 220) are spread apart from each other while their parallel orientation is maintained. The lifting body (210) is connected to a lifting arm and lifted causing the retraction rods to lift and support the abdominal wall.

摘要： A method and apparatus for mechanically lifting the abdominal wall (W) away from underlying abdominal organs (C) for laparoscopic surgery without insufflation. In the method an expansible device (74) is inserted in the abdominal cavity through a small incision in a collapsed state and then expanded into engagement with an extensive area of the abdominal wall. Lifting force is then applied to the device for peritoneal retraction. The device takes the form of mechanical rods or arms and/or balloons. In the balloon embodiments lifting force may be applied externally of the abdominal cavity, or internally of the cavity by balloon inflation. Certain of the balloon embodiments are of an annular or U-shaped configuration and include a membrane for draping the internal organs and/or a centrally located balloon for lateral expansion. The balloons may be provided with an internal endoscope for viewing. The method also provides for laparoscopic gallbladder removal, either to the interior or exterior of the balloons. A needle (144) is provided to laparoscopically pierce and drain the gallbladder. The needle carries a balloon (146) inflatable to grip the gallbladder for rectraction and removal.

摘要： A method and apparatus for removing a pre-placed prosthetic joint from a bone cavity and conditioning the cavity for receipt of a replacement joint. The pre-placed joint (64) is first pulled from the mantle (66) of hardened cement holding it within the cavity, thus leaving a cavity within the mantle (66). A mass of fluid cement is then placed within the cement cavity, after which a pulling appliance (48) is inserted into the fluid cement and the fluid cement is permitted to cure to bond the appliance (48) to the mantel (66). Tension is then applied to the pulling appliance (48) to remove the appliance (48) and the cement mantle (66) from the bone cavity as a unit. The fluid cement partially dissolves and softens the hardened cement mantle (66) so that an integral cement mass is formed upon curing of the fluid cement. The pulling appliance (48) is of a screw-like configuration, with means to connect a slap-hammer (22) to its proximal end.
