    • The invention is directed to a suture clinch that may be placed upon suture extensions of a placed suture to securely lock the extensions in a desired location. The clinch operates to replace a tied knot and retain the suture in a fixed relationship with little or no damage to the suture itself. The clinch comprises a mating pair of interlocking members that restrict movements of the suture. The interlocking members may be placed upon the suture extensions from alongside the suture extensions and compressed to a first condition where the interlocking members may be advanced, retracted or adjusted, and subsequently further compressed to a second condition where the interlocking members are fully engaged to fully restrict the movement of the suture. In another aspect, the clinch may be provided in a flat form comprising a first component and a second component. The first and second components fit together in a mating relationship that entraps at least a portion of the suture between tractive faces of the first and second components, respectively. Each end of the first and second components may have a recessed central portion that leaves two extensions along the sides of each end of the components. The extensions of the second component are subsequently folded over the first component to provide secure entrapment of the suture between the tractive faces of the first and second components. The tractive faces operate to engage the suture and hold it securely in place without damaging or challenging the suture. The clinch may comprise of plastic, metallic materials such as stainless steel or titanium, or a combination of both where the plastic is chosen for its value as an atraumatic interface and the metal for its radiopacity or durability.

    • The invention is directed to a device and method for securing a passed surgical suture in a manner similar to a tied knot. The suture securing device comprises a male piece having a top portion, a center shaft having one end connected to the top portion, and an end member having a capturing feature connected to another end of the center shaft; and a female piece having a top end and a bottom end comprising a column being generally cylindrical in shape and forming a hollow opening along its length, wherein the opening slidingly receives the center shaft and a suture to be captured and held by the capturing feature, the suture being secured in place between the column and the center shaft. The assembled male and female pieces are irreversibly interlocked and form a lightly compressed, convoluted or tortuous pathway. The capturing feature is formed to engage and obliquely capture the suture from different angles. The radii of the male and female pieces are preferably greater than the radius of the suture. The securing device may further comprise a link formed between the male and female pieces. The link may be a tether formed when the male and female pieces are formed. The link may be severed and removed after placement and assembling of the male and female pieces. In another aspect of the invention, a method for applying a suture securing device is disclosed comprising the steps of: (1) providing the suture securing device of the invention; (2) aligning the male piece and the female piece at a desired location for applying the suture securing device; and (3) pressing the center shaft of the male piece into the opening of the female piece. The method may further comprise the step of applying a compressive load on the male piece to dislodge an enlargement from an undercut of the female piece and to place the male piece and the female piece in a condition where they may be advanced to a desired position along the suture.

    • A process of making a tack-free gel is disclosed comprising the steps of providing a mold defining a mold cavity, the mold cavity comprising a plastic material; pouring or injecting a molten gel having a high molding temperature into the mold cavity; and forming the tack-free gel as a thin layer of plastic of the mold cavity is melted over the gel. The forming step further comprises cooling the gel from the molten state to a solidified state. The melting temperature of the plastic material is lower than the molding temperature of the gel; and the higher the temperature differential, the greater the melting of the plastic material and the thicker the layer of the plastic material on the surface of the gel. The mold may be formed of low-density polyethylene (LDPE). With the process of the invention, the heat of the molten gel at its molding temperature is transferred to the surface of the LDPE mold so as to melt a thin layer of the LDPE. The mold may comprise a mold base having a plurality of mold holes forming a plurality of mold cavities, each of the mold holes comprising an axial pin to mold an axial hole through a center of the gel, an LDPE cylinder providing a predetermined inside diameter for the mold, and an LDPE disc mounted on the axial pin and disposed at the bottom of each mold cavity in the mold base. The process may further comprise the step of dabbing the gel in a low-friction powder such as polytetrafluoroethylene (PTFE) and a lubricant. The mold may further comprise a mold top disposed axially of the mold base and comprises a plurality of holes forming a plurality of cavities, each of the mold top holes is adapted to receive the LDPE cylinder, and a second LDPE disc disposed at the top of each mold cavity of the mold top.

    • Clip appliers to apply a surgical clip and methods concerning the same are provided. One provided applier has a cartridge and an actuator. The rack mechanism causes a feeder to move proximally as a closure member moves distally, such that jaw members move together to crimp a clip there between. The rack mechanism also causes the feeder to move distally as the closure member moves proximally, such that a crimped clip is released and another clip is fed into the jaw members. The rack mechanism further includes a rack connected to the closure member in which the closure member and the rack are arranged to move synchronously throughout the releasing of a crimped clip and feeding of another clip, e.g., an opening stroke.
