摘要： Various apparatus and methods for excising tissue are described therein. In certain variations, a method of excising a target tissue mass from a subject, may include advancing an elongate instrument having an opening at its distal end into a subject. A cutting element, e.g., a snare or loop, may be advanced from the distal end of the elongate instrument, wherein the cutting element may be configured to cut tissue. The cutting element may be positioned near the target tissue mass. The cutting element may be actuated, e.g., expanded or deployed, and may perform energy based or mechanical cutting of a section of tissue encapsulating the target tissue mass. The cut section of tissue and the target tissue mass encapsulated therein may be removed from the subject.

A transducer coupled to the elongate shaft can be configured to deliver mechanical energy to the elongate shaft, e.g., axially along the elongate shaft, to rotate, vibrate, and/or flex the elongate shaft to facilitate dissection of tissue adjacent at least a leading end or a distal end of the elongate shaft. In some embodiments the elongate shaft can include at least two coaxial coils, and the transducer can be configured to move at least one of the coaxial coils to facilitate tissue dissection using the elongate shaft.

摘要： An endoluminal treatment device and method includes introducing an endoluminal device into a lumen of a patient and engaging a wall of the lumen with a retractor of the endoluminal device. The wall is retracted such that outer surfaces of the lumen come into contact with each other and thereby form a plication. Following formation of the plication, the device is activated to deploy a securing device to secure the plication. Multiple plications can be introduced into the organ wall and then be approximated to reduce the volume of the lumen and to form a pouch, such as in a gastric reduction procedure.

摘要： An RF treatment device according to the present invention includes one or two radio frequency electrode holders each having multiple needle electrodes arranged to create a transmural lesion through tissue, with or without the presence of fat on the tissue. When inserted through the tissue, the needle electrodes are in single straight or curved line with the needle electrodes being parallel to each other, adjacent electrodes having opposite polarity, and having tips and shafts designed to minimise tissue penetration force. Lubricious coatings may be provided for this purpose, along with or in lieu of motion imparting mechanisms which to impart vibration, oscillation, or rotation which facilitate penetration. Further, RF energy may applied during penetration for this purpose. The embodiments are described for use in creating a transmural lesion in heart tissue to treat atrial fibrillation by blocking the passage of abnormal electrical currents through the heart. However, the devices and methods may also be used to create continuous transmural lesions in heart tissue and other body organs and tissues to treat other conditions. Although discussed in the context of the application of RF bi-polar energy, other types of energy, such as cryo, ultrasound, microwave, heat, or other electrical means of providing an interruption to the abnormal electrical circuits at given regions can be used.
