SOLUTION: The apparatus includes one or more electrodes 27 and 29 which apply electrical signals to a cardiac muscle segment, signal generation circuitry 26 configured to generate stimulation including at least one of excitatory stimulation and non-excitatory stimulation, which is supplied to the one or more electrodes, and control circuitry to control the signal generation circuitry 26 to generate the excitatory stimulation for heart pacing and the non-excitatory stimulation for increasing cardiac output at a time, wherein the non-excitatory stimulation is prevented from generating action potential propagated to the heart, when it is applied to the heart.

SOLUTION: This apparatus for modifying cardiac output of the heart of a subject, includes: one or more sensors 29 for sensing signals responsive to cardiac activity; and a stimulation probe 21 including one or more stimulation electrodes which apply non-excitatory stimulation pulses to a cardiac muscle segment. Signal generation circuitry 22 is coupled to the one or more sensors and the stimulation probe, which circuitry receives the signals from the one or more sensors and generates the non-excitatory stimulation pulses responsive to the signals. 摘要： During a cardiac arrhythmia, defibrillation shocks from an implanted cardiac defibrillator are suppressed based on the sensing of electrical wavefront arrival times which indicate a supraventricular origin to the cardiac arrhythmia. In some embodiments, times of electrical wavefront arrival in at least two ventricular locations of the heart are sensed; one location being relatively superior, and one relatively inferior (e.g., relatively superior and inferior locations of a ventricular septum). In some embodiments, if the arrival time at the more inferior position is within a predetermined interval after arrival at the more superior position, delivery of defibrillation shocks are suppressed. In some embodiments, additional sensing of electrical wavefront arrival at one or more non-septal ventricular locations is performed, and defibrillation shock suppression is optionally itself suppressed if the additional sensing indicates that the wavefront initiated in a ventricular location.
