    • PROBLEM TO BE SOLVED: To provide a method and a system for classifying cardiac response to a pacing pulse and avoiding retrograde cardiac conduction.SOLUTION: This cardiac system includes a controller that performs retrograde management. The retrograde management includes the steps of: sending out a first atrium pacing pulse to an atrium during a cardiac cycle; detecting a P wave during a post-ventricular atrial refractory period of the cardiac cycle; determining whether or not an interval between the detected P wave and a scheduled time of a second atrium pacing pulse exceeds an effective refractory period of the atrium; delaying the second atrium pacing pulse when the interval exceeds the effective refractory period of the atrium; sending out the second atrium pacing pulse after the effective refractory period of the atrium; determining whether or not the second atrium pacing pulse captures the atrium; and increasing the length of the effective refractory period of the atrium when the second atrium pacing pulse does not capture the atrium.

    • PROBLEM TO BE SOLVED: To realize a plurality of approaches to provide a timing optimal for a ventricular pacing by judging internal electrical or mechanical events having a predictable timing relationship with the sending of the ventricular pacing with the optimal timing that allows ventricular functions to be maximized. SOLUTION: This pacing system is to provide an optimal hemodynamic cardiac function for parameters such as contractility (peak left ventricle pressure change during systole or LV+dp/dt), or stroke volume (aortic pulse pressure) using system for calculating atrio-ventricular delays for optimal timing of a ventricular pacing pulse. The system provides an option for near optimal pacing of a plurality of hemodynamic parameters. The system uses the proper timing using electrical or mechanical events having a predictable relationship with an optimal ventricular pacing timing signal so as to induce its proper timing. • PURPOSE: To realize accurate discrimination, by comparing a vector of reference line obtained from each sequence of characteristic values of a group of bioelectric potentials with a vector of non-reference line when a ventricular tachycardia is discriminated from a non-ventricular tachycardia on the basis of bioelectric potentials of the heart. CONSTITUTION: Bioelectric potentials of the heart are detected and sent to a signal processor 12, and output signals from this signals processor 12 are input into a discriminating algorithm 13 to carry out an algorithm analysis. On the basis of results obtained from the algorithm analysis, a prescribed medical treatment is carried out. For example, a pulse generator 11 is made activated to give a shock to the heart with a cardioversion. The discriminating algorithm 13 compares characteristic values of heart-related signals determined as the non-reference line with characteristic values of heart-related signals set up as the normal reference line, to discriminate the heart tachycardia. The tachycardia is discriminated by placing the discriminating point at the coordinates in the discriminating plane which is equal to an analogous value of the non-reference line vector normalized against the normal reference line vector and is also equal to an immovable value.

    • PURPOSE: To provide a cardiac pacemaker with an automatic pulse rate response gain factor responsive to at least one cardiac-related physiological characteristic of a patient. CONSTITUTION: A rate controller (I) in a pulse generator detects respective values of cardiac-related physiological characteristic of a patient for specified time intervals. Each measurement is a mean value of detections over the multiple incremental time intervals. I retains the maximum measurements in the incremental time interval, averages the maximums, and controls pulse rate response gain factor in relation to the resultant average measurements and a specified target pulse rate. Preferably two physiological characteristics should simultaneously be measured, where the first parameter should have a threshold level requiring arrival of the signal before capture of the second parameter for subsequent averaging, for example, those parameters are accelerometer values and time cardiac outputs.
