    • An endoscopic surgical instrument for deploying a two part fastener having a male fastener part and a female fastener part, includes a tube having a proximal end and a distal end. An end effector is coupled to said distal end of said tube and is configured to hold the male fastener part and the female fastener part in opposed relation. An actuator is coupled to said proximal end of said tube and is configured to actuate the end effector. A shearing device may be positioned proximate the end effector to shear off a tip of the male fastener part after the male and female fastener parts are moved into locking relation. In addition, the surgical instrument may include a male fastener part having a shaft with a plurality of detent positions. Further, the male fastener part may include a base and a shaft pivotally connected to the base.

    • A multifilar flexible rotary shaft includes a plurality of individual filaments which are not wound around each other or around a central core, a loose ensemble of filaments. The input ends of each filament are coupled to each other and the output ends of each filament are coupled to each other. Preferably all of the filaments are identical. A loose ensemble of N filaments can transmit N times the torque of a single filament, and will have N times the torsional stiffness of a single filament, while retaining the minimum radius of operation of a single filament. Since a loose ensemble of filaments does not have any appreciable contact forces among the filaments (because they are not forcibly twisted together), there is no appreciable internal friction or hysteresis. In order to appreciably eliminate all the hysteresis in such an ensemble of filaments, the filaments should be no more than loosely twisted together, if at all. The only reason for loosely twisting the wires together is to allow the ensemble to be easily handled and to insure that the individual wire filaments follow the same general curved path from one end to the other. Several practical applications of the invention are also disclosed.

    • An endoscopic needle device includes an outer sheath, an elongate shaft extending through the sheath, a needle rigidly attached to the distal end of the shaft, and a handle assembly having a housing coupled to the proximal end of the sheath and having a syringe port in fluid communication with the sheath and a knob rotatably coupled to the housing. Rotation of the knob relative to the housing causes rotation of the shaft relative to the sheath. The needle and the distal end of the sheath are mated by a threaded connection. As such, rotation of the knob relative to the housing causes precise longitudinal movement and extension of the needle relative to the distal end of the sheath. A set of intermeshing gears are provided between the knob and shaft which function to permit at most one rotation of the knob to cause complete extension and/or retraction of the needle. Furthermore, assemblies are provided with permit the shaft to accommodate movement of the needle relative to the sheath. Moreover, the position of the knob relative to the housing provides an indication of the extension of the needle. As such, the injection needle device of the invention permits controllable and calibrated rotation of the needle to within a few degrees, such that the needle device can be easily, reliably, and precisely operated to advance the needle.
