摘要： A myocardial pacing lead includes two sets of at least two tines placed to oppose each other. The lead is pressed against the epicardium and is rotated until a rough porous tip abrades the epicardium and allows the tip and the first row of tines to penetrate the myocardium. Excessive penetration and lead migration is prevented by the second row of tines which is mounted backward and just proximal from the first row of tines. The first row of tines anchor on the inside surface of the epicardium which prevents dislodgment. Dacron pads on the second row of tines assures chronic fixation by adhering to the outer surface of the patient's epicardium.

Each rate control parameter has a value which varies as a function of changes in a patient's physiologic demand an includes a sensor system for sensing the rate control parameter value and for providing a sensor output representative thereof. The cardiac pacemaker also includes control circuitry which includes a rate response defining means for deriving desired pacing rates as a function of the sensor output and an achievement monitoring means that has a predetermined achievement criterion, for monitoring the relationship between the derived pacing rates and the achievement criterion over an achievement output. An output circuitry provides optimized pacing rates as a function of the desired pacing rates; and a rate response control means adjusts the rate response defining means accordingly.

The system determines a plurality of cardiovascular pressure metrics based on the measured cardiovascular pressure signal, including at least one cardiovascular pressure metric indicative of a timing of at least one cardiac pulse. The system also determines a metric indicative of a timing of at least one heart depolarization within the measured electrical depolarization signal. The system compares the timing of the at least one cardiac pulse to the timing of the at least one depolarization, and determines whether to discard the plurality of cardiovascular pressure metrics based on whether the timings substantially agree.

An estimated arterial diastolic pressure is based upon an amplitude of the sensed ventricular pressure corresponding to a time at which a first derivative of ventricular pressure as a function of time is at a minimum (dP/dtmin). Fill parameters such as isovolumetric relaxation constant, ventricular suction pressure, atrial kick pressure, and transvalve pressure gradient are derived from measured pressures representing minimum ventricular pressure, ventricular diastolic pressure, and diastasis pressure.

Through use of the one or more modes of statistical analysis, a medical professional can be aided during evaluation of patient data for diagnosis of the patient. At least one of the monitored parameters may include one or more values used representatively for storage intervals of a selected length. As such, for each storage interval, a value may be determined for the one monitored parameter occurring at an upper percentile and a lower percentile. In addition, a median value may be determined for the one monitored parameter for each storage interval. Over a plurality of the storage intervals, these determined values can be used in one or more modes of statistical analysis to better identify and assess the arrhythmias.
