A preferred embodiment of the inventive method of forming the continuous sheet (106) is disclosed wherein at least one rotatable mechanical web guide (72) is used to align the webs (38,58) in the edge-to-edge relationship, thereby enabling precise alignment not available with existing optic-type web guides. A preferred embodiment of the printable business form product (10) is disclosed wherein heat-resistant tape (18) enables the business form product (10) to be printed in high-heat printing applications (e.g., printed in an offline laser printer) without fracturing the taped seam (16).

At least one of the labels is dimensioned and configured to hold a removable cap on a medical container to indicate if the container cap has been tampered with, and preferably that label has a length of between about 10-11 inches so that it can be wrapped completely around a 90 ml plastic specimen vial, or other medical container, so that portions of the label are in contact with each other, enhancing the tamper evident functionality of the label. A second label on the release liner may be a box seal, and other labels may be provided to facilitate tamper evident functionality of the form.

摘要： A multi-ply form with labels and multiple form parts is provided, including first and second webs secured one to the other with repositionable adhesive. A plurality of different types of labels are permanently secured to the second web. At least one label of the second web registers with a die-cut portion in the second web and both the one label and die-cut portion register with a die-cut section of the first web. When the webs are separated, the die-cut section is adhered to the second web, enabling removal of the one label therefrom in a manner to provide securment to a surface using either repositionable adhesive or permanent adhesive. Additional holding power may be provided when using the repositionable adhesive by providing a further die-cut segment within the die-cut section of the first web whereby the area of application of the adhesive may be large or small, depending upon whether the die-cut section is removed or not. The first web also includes a second section forming a first form part. A second form part overlies this first form part.
