摘要： This invention is an apparatus for tissue dissection and instrument anchoring, including a dissection balloon (512) having a viewing window (508) at its distal end, or including an anchoring dissection balloon having a non-uniform elasticity selected to achieve desired inflation shape and pressure characteristics. The window, which can be a wide angle lens, is transparent and sufficiently strong to retain a desired optical shape while being pushed against tissue layers by a rigid instrument (515) deployed within the balloon. In preferred embodiments, the instrument includes a long necked dissection balloon deployed through a cannula, and a housing (513) to which the dissection balloon's mouth is attached. In accordance with a method of use of the invention, the distal end of the balloon is inserted between tissue layers and inflated to dissect the tissue layers.

摘要： An apparatus and method for cauterizing the endometrial tissue of the uterus includes an inflatable balloon (10) mounted to the end of a cannula (20) and spring members (50, 52) which shape the balloon (10) to approximate the bicornual shape of the uterus. A heating coil (90) heats a saline solution to approximately 190-210 DEG F and a pump (100) circulates the saline solution through the cannula (20) and inflates the balloon (10). The inflated balloon (10) contacts substantially all of the intrauterine surface and is held in contact with the surface for 5 to 7 minutes until the tissue has been necrosed. A fiberoptic scope (135) is used to monitor balloon placement and cauterization effect.

摘要： An apparatus for separating a first layer of tissue, such as the peritoneum, from a second layer of tissue, such as the properitoneal fascia. The apparatus includes a main envelope that defines a main inflatable chamber (113). The apparatus also includes an introducing device (160) that introduces the main envelope in a collapsed state between the first layer of tissue and the second layer of tissue. The introducing device also inflates the main envelope into an expanded state to separate the first layer of tissue from the second layer of tissue, and to create a working space between the two layers of tissue. Finally, the apparatus includes an insufflating device (125, 131) that introduces insufflation gas into the working space between the two layers of tissue. A method for separating a first layer of tissue from a second layer of tissue provides a main envelope and insufflation gas. The main envelope defines a main inflatable chamber. The main envelope is introduced in a collapsed state between the first layer of tissue and the second layer of tissue. The main envelope is inflated into an expanded state to separate the first layer of tissue from the second layer of tissue, and to create a working space between the two layers of tissue. 摘要： An apparatus and method for separating tissue layers and providing a working space for performing a medical procedure. A first balloon is inserted between the tissue layers and inflated to dissect the tissue layers. A second balloon is also positioned between the tissue layers and inflated to retract the tissue layers. The first balloon is then deflated and punctured with a sharp instrument to create an opening in the first balloon. The second balloon is positioned to impede the escape of insufflation fluid from the working space and the working space is then insufflated. An instrument for performing the medical procedure may then be introduced into the working space through the opening in the first balloon.

摘要： A first inflatable retraction device has a first inflatable chamber (5) and a non-pressurized chamber (13) inside the main chamber. The non-pressurized chamber is expanded by inflating a second inflatable chamber (25). The non-pressurized chamber enables the main chamber to remain inflated when an aperture is cut in the envelope of the main chamber, through which treatment is carried out. A second inflatable retraction device has an inflatable retractor (103) and a maintainer (107). The inflatable retractor retracts the organ and the maintainer maintains the organ in its retracted condition after the inflatable retractor is deflated. The maintainer can be inflatable, and can be inside or outside the inflatable retractor. A self-retracting endoscope (201) has an optical assembly with an expandable retractor (211) fitted to its distal end. The distal end of the endoscope is inserted into the body with the retractor in a collapsed condition. The retractor is then expanded to retract organs that would otherwise obstruct the view from the distal end of the optical assembly. After observations are complete, the retractor is returned to its collapsed condition. An insertion tube (243) enables cylindrical objects, such as packaged inflatable retraction devices, to be pulled, instead of pushed, into the body. The additional chamber of an inflatable retraction device having two inflatable chambers is filled with a slurry of a particulate solid in a liquid. The liquid is removed and the additional chamber evacuated to consolidate the particulate solid. This increases the retracting strength of the additional chamber.

摘要： A family of retraction devices for use in laparoscopic surgery. The retraction device is introduced into a body cavity in a collapsed condition, and, once in place, is expanded to an expanded condition to retract an organ. Access to the tissue to be treated is provided through the retraction device. Most of the retraction devices are inflatable, having a main inflatable chamber, a means for selectively inflating the main inflatable chamber (11) into an expanded condition, and a maintaining means (e.g., 31) for maintaining the main inflatable chamber in its expanded condition after an aperture (54) is pierced in the main chamber to provide access to the tissue. 摘要： A method and apparatus for creating a surgical work space by lifting the abdominal wall and retracting abdominal organs during surgery, utilizes a lifting body having two or more retraction rods supported by the lifting body. The rods are designed to be introduced into the abdominal cavity through a laparoscopic puncture opening and to be extended within the abdominal cavity to provide a lifting area. In the preferred embodiment, a pair of retraction rods (212, 220) disposed with the bores and substantially parallel to each other, are inserted into the abdominal cavity through an incision in the abdominal wall. The retraction rods (212, 220) are spread apart from each other while their parallel orientation is maintained. The lifting body (210) is connected to a lifting arm and lifted causing the retraction rods to lift and support the abdominal wall.
