    • An apparatus for exchanging over-the-wire balloon catheters engages a guide wire within a guide catheter. In one embodiment the engagement is by an inflatable balloon in the guide catheter. Alternatively, the engagement is accomplished by a captivation wire in the guide catheter that has a collapsible loop portion through which the guide wire extends. Inflation of the balloon or closing the loop portion of the captivation wire within the guide catheter traps the guide wire and restricts its movement relative to the guide catheter. Once the guide wire position is fixed, withdrawal of a first balloon catheter and subsequent introduction of a second balloon catheter over the guide wire is possible without moving the guide wire longitudinally. Thus, the positioning of the guide wire over a stenosis to be dilated is not disturbed.

    • A catheter (4) includes a hollow catheter shaft (18), an inflatable balloon (36) surrounding the catheter shaft distal portion, and an expandable endoluminal prosthesis, such as a stent graft (38, 40), surrounding the balloon. The stent is typically a coiled stent having distal ends (58, 48) slidably housed within stent end holders (56, 50) on the catheter shaft. The first stent end is released from its holder by inflating the balloon. The second stent end is selectively released from its holder by other than inflating the balloon, typically by a push wire (30) pushing the second stent end out of its holder. A sheath (8) is moveable between a first position surrounding the stent and a second position spaced-apart from the stent. The stent is prevented from assuming its expanded-diameter state by (1) the engagement of the stent ends with the catheter shaft through the stent end holders, and (2) the sheath being in its first position during the introducing step. The stent may be a straight stent (3 8) or a bifurcated stent (38D).

    • A stent delivery system for medical treatment of a patient includes a balloon catheter, a stent, a sheath, and a flexible tapered introducer tip. The balloon catheter has a hub, shaft, and inflatable balloon, as well as a tubular stent mounted about the deflated balloon and crimped to an initial diameter. The introducer tip is affixed to the distal end of the balloon catheter and tapers to a maximum outer diameter equal to the crimped stent outer diameter plus the sheath wall thickness. The balloon catheter shaft defines a groove along a major portion of its length, in which a guidewire may be partially received or embedded. This grooved design enables the stent delivery system shaft to have a smaller diameter. The composite sheath has a multiple diameter design with a distal portion adapted to retractably cover the crimped stent, and a proximal portion having a smaller diameter. This smaller diameter enables greater flow of a radiopaque contrast fluid around the outside of the proximal sheath and through the inside of a guiding catheter, thus providing the system with a high visibility capability. The introducer tip defines a shoulder against which the sheath can push in a distal direction, assisting the stent delivery system to perform primary stent delivery without a preliminary angioplasty procedure.

    • An active cannula or sleeve which does more than merely maintain a channel or passage is usable to create and/or enlarge a channel or passage, to position a scope or instrument, to move or locate tissue, etc. The cannula can vary in size or shape as needed, intraoperatively. Because a cannula of the present invention is expandable, the surgeon can make a small relatively small incision, stretch the tissue with the expandable cannula, contract the cannula and remove it, allowing the skin to come back to its unstretched condition. Thus, a smaller incision can be made to fit the same size instrument. This results in less trauma and scarring and an easier operation. The cannulas are or can assume such a non-circular shape, to fit into a natural skin opening and cause less trauma. The devices can be used to seal off a space; to expand an existing space or a potential space for working or visualization; to move tissue (for example, to stretch an incision) or to protect it.
