摘要： An apparatus and method preventing cardiac arrhythmias with pacing therapy is disclosed. Upon detection of a pre-arrhythmic condition, an implantable cardiac rhythm management device is configured to deliver pacing therapy in a manner that pre-excites particular myocardial sites that have been identified as stressed sites and likely locations for the origination of arrhythmias. Such pacing results in a reduction in myocardial wall stress at those sites during systole and reduces the probability of an arrhythmia occurring.

摘要： A catheter with a tissue property sensor provides for localization of myocardial infarction (MI) by utilizing one or more differences between properties of infarcted myocardial tissue and properties of normal myocardial tissue. The tissue property sensor is to be placed on endocardial wall or epicardial wall during catheterization to sense at least one tissue property allowing for detection of MI. Examples of the tissue property sensor include, but are not limited to, an optical sensor, an acoustic sensor, a contractility sensor, a temperature sensor, and a drug response sensor. In one embodiment, the tissue property sensor senses a tissue property in various locations on endocardial wall or epicardial wall and detects substantial changes in the tissue property that indicate a boundary between infarcted tissue and normal tissue.

摘要： An implantable device for delivering phototherapy is described that enables the phototherapy to be delivered to internal locations in either a clinical or ambulatory setting for treatment of post-MI patients. Telemetry circuitry enables the device to deliver the phototherapy upon command or be programmed to delivery the phototherapy according to a specified schedule. The device may also incorporate one or more sensing modalities that can be used to trigger delivery of phototherapy upon occurrence of a sensed event or condition. In one particular embodiment, the phototherapy device is incorporated into a cardiac rhythm management device that also delivers pacing and/or defibrillation therapy.

Linear stimulation may be accomplished using a linear source, which may comprise an elongated electrode or plurality of electrodes arranged so as to linearly stimulate the heart. A linear source may be coupled to the left ventricle, and one or more additional electrodes may be positioned so as to stimulate a separate region of the left ventricle or one or more additional chambers of the heart as well. Application of an electrical stimulus by the linear source may function to stimulate a larger region of the heart, thereby enhancing the resynchronization effect as well as providing other hemodynamic benefits.
