    • A variety of methods for extracting different substances such as endogenous substances, pharmaceutical substances, markers of disease, and their metabolites across a tissue are provided. The methods utilize an AC signal to maintain a substantially constant electrical state in a region of the tissue through which extraction occurs, thereby allowing substances to be transported across the tissue in a controlled and predictable manner. Certain methods include an optional AC or DC prepulse signal to initially achieve the target electrical state. An optional DC offset signal can also be utilized to assist in promoting extraction of the substance. The methods have utility in a variety of different clinical settings and applications.

    • The present invention relates to an apparatus and method for localized electroporation of tissue. An apparatus includes a perforate electrically insulating layer, a first electrode at a first side of the perforate electrically insulating layer and a second electrode at a second side of the perforate electrically insulating layer. An electric field extending between the first and second electrodes will preferentially extend through perforations of the electrically insulating layer. The electric field thereby causes electroporation of tissue that is proximate to the first electrode and is partitioned from the electrically insulating layer and the second electrode by the first electrode. The apparatus controllably limits the depth of the electric field within a tissue, such as skin, thereby electroporating a surface layer, such as a stratum corneum layer of the skin, without stimulating submerged nerve endings within the skin. Sensation of the applied voltage and especially pain sensation can thereby be controlled by the invention.

    • An iontophoretic drug delivery device of the present invention includes a power source and an electrode assembly containing at least two electrodes, in electrical contact with a first reservoir and a second reservoir. The present invention also includes at least three electrodes in a three electrode embodiment, with the third electrode in electrical contact with a third reservoir. In either embodiment, the first reservoir and the second reservoir contain an active agent to be delivered to an applied area of a patient. The present invention also includes in its two electrode embodiment a switch for reversing the flow of current or in its three electrode embodiment a switch for switching the flow of current, so as to reduce sensation. The devices also include a timer for controlling the amount of time the electrical current flows in each direction. In this way, the devices are suitable for use to deliver an active agent to the applied area approximate the first reservoir when the electrical current flows in one direction for a first period of time and to deliver an active agent to the applied area approximate the second reservoir when the electrical current flows in the other direction during a second period of time. Thus, reversing polarity or switching the flow of current between electrodes, and iontophoresing thereunder below the sensation threshold results in sensation anesthesia so that the current can be raised to deliver the drug without sensation.

    • Methods for conducting controlled environment electrotransport are described. The methods generally concern control of the ionic environment of the active electrode reservoir, in an electrotransport system. This control may be utilized, for example, to maintain a constant extraneous ion presence, such as pH. It may also be applied in a preferred manner to maintain selected control over the delivery rate of a target species, in time, in the presence of extraneous ions. Apparatus usable to provide for controlled environment electrotransport are also described. In general the apparatus comprises an electrotransport system including an active electrode reservoir having therein both a primary electrode arrangement and a secondary electrode arrangement, with a control arrangement for selected operation of both. The secondary electrode arrangement, being operated different from the primary electrode arrangement, is selected for operation to selectively enhance or otherwise modify ion presence in the active electrode reservoir, to achieve application of the general methods described. The apparatus may include a sensor means therein, for use as feedback to a control system during operation. Methods and systems involving diffusion type delivery are also described.
