摘要： Prostate treatment using fluid stream to resect prostate tissue, thereby relieving symptoms of conditions such as BPH, prostatitis, and prostatic carcinoma. A device having a fluid delivery element is positioned within a lumen of the urethra within the prostate. A fluid stream is directed outwardly from the fluid delivery element toward a wall of the urethral lumen. The fluid delivery element is moved to scan the fluid stream over the wall to remove a volume of tissue surrounding the lumen. The fluid may be combined with therapeutically active substances or with substances that increase resection efficiency. Fluid force may be adjusted to provide selective tissue resection such that soft tissue is removed while harder tissue is left undamaged. In order to gain a working space within the urethra, another fluid may be introduced to insufflate the urethra in the region of treatment.

摘要： Devices and methods are provided for the ablation of regions of the digestive tract to achieve hemostasis and to eradicate chronically bleeding lesions as occur with gastric antral vascular ectasia (GAVE), portal hypertensive gastropathy (PHG), radiation proctopathy and colopathy, arteriovenous malformations, and angiodysplasia. Ablation is typically provided in a wide-field manner, and in conjunction with sufficient pressure to achieve coaptive coagulation. Ablation, as provided the invention, starts at the mucosa and penetrates deeper into the gastrointestinal wall in a controlled manner. Ablation control may be exerted by way of electrode design and size, energy density, power density, number of applications, pattern of applications, and pressure. Control may also be provided by a fractional ablation that ablates some tissue within a target region and leaves a portion substantially unaffected. Embodiments of the device include an ablational electrode array that spans 360 degrees and an array that spans an arc of less than 360 degrees.

The anchor can comprise a balloon-type device, a stent-like device, a strut-like device, a spring-strut-like device, an umbrella-like device, a mushroom-like device, or other device that allows the catheter to maintain a position with respect to target tissue. The ablator can comprise a balloon ablator, an umbrella ablator, a pinwheel ablator, an umbrella ablator incorporating a cinch mechanism, a mushroom balloon ablator and a segmented balloon or pinwheel ablator. The anchor and ablator can also comprise a combination mushroom balloon anchor section and mushroom balloon ablator section. The anchor and ablator can include electrodes for measuring a conductance therebetween when in deployed position, so as to determine the effectiveness of the ablation.

The catheter has one or more balloons located near the distal end to enhance effectiveness of the sclerosant and prevent its delivery to the deep venous system. The catheter has an orifice located near the distal end of the catheter to direct the sclerosant into the vessel and one or more effluent openings to provide for removal of the sclerosant fluid. A heating member such as an electrical resistance element, a Laser probe, or an RF electrode located within the catheter lumen or on the outside of the catheter shaft heats the sclerosant fluid to improve its ablation effectiveness.
