    • The invention relates to a surgical instrument for at least partially occluding a body conduit comprising a first jaw, a second jaw opposing the first jaw and moveable relative to the first jaw to occlude the conduit, and an insert being mounted by at least one of the first and second jaws with a length of the insert extending along at least one of the first and second jaws. One of the first and second jaws may further comprise a retention barb to provide a friction fit with the insert. The insert is flexible, disposable and may be formed by injection molding. The retention barb may be a three-sided or multiple-sided barb that is formed at the proximal end of at least one of the first and second jaws. The first and/or second jaws may be bent and/or curved. In another aspect, the insert comprises an outer wall and an inner wall for fitting over the jaw, and the inner wall further comprises a molded, rectangular raised ring formed at the proximal end to aid the retention of the insert over the retention barb of the jaw. The insert may be removed from the jaw by pulling the insert in a distal direction to overcome the friction fit of the retention barb. The insert may further include a soft tip covering the distal end of one of the jaws to minimize trauma during surgery. The distal ends of the jaw and the insert may be tapered to allow ease of installation and removal. In another aspect, the insert and jaws may be removably attached to one another in a snap-fit relationship where the jaws may include one or more holes and the insert may include one or more projections. It is appreciated that the holes and projections may vary in width, depth and height depending on application.

    • The invention is directed to a trocar assembly having a channel defined along an elongate axis, the trocar assembly being adapted to receive a surgical instrument, the trocar assembly comprising a septum seal disposed in the channel including a seal tip having a proximal facing surface, the seal tip including portions defining an orifice; and a septum shield including a tubular member having a proximal end and a distal end, and a plurality of blades or leaflets protruding distally from the distal end of the tubular member, the septum shield being placed inside the septum seal such that the blades engage the proximal facing surface of the seal tip. The trocar assembly may further comprise a zero closure seal such as a double duckbill valve disposed in the channel outside of the septum seal. The septum shield operates to reduce the drag force and to minimize axial movement of the septum shield and the instrument during insertion and removal of the instrument through the septum seal. The septum shield may be formed from a rigid plastic material, and the septum seal may be formed of an elastomeric material. The blades or leaflets may overlap or offset one another. The orifice may be expandable to accommodate the instrument having a diameter of about 5-15 mm. The blades or leaflets have distal tips that glide or roll against the instrument during placement of the instrument. The distal tips may comprise of a combination of material, durometer and shield geometry to control the behavior of the septum shield. The trocar assembly may further comprise a second septum shield disposed outside of the septum seal.

    • The invention is directed to a trocar seal adapted to form a seal around a surgical instrument, the trocar seal comprising a housing having an axis extending between a proximal end and a distal end and forming a working channel sized and configured to receive the surgical instrument; an elastomeric septum seal having a proximal face and a distal face, the septum seal being disposed in the housing and extending transverse to the axis of the housing across the working channel; portions of the septum seal defining a hole having a diameter less than or equal to the diameter of the surgical instrument so that during insertion of the instrument along the working channel the septum seal forms a seal with the instrument; and at least one structure formed integrally with or proximate to the septum seal to prevent inversion of the septum seal upon withdrawal of the surgical instrument. The septum seal or the housing may further comprise a sidewall to include the structure extending from the sidewall to the distal face of the seal to tether the seal distally. The structure may be integrally formed with the proximal face of the septum seal to reinforce the seal, or the structure may be integrally formed with the distal face of the septum seal to tether the seal distally. The structure may be an annular reinforcement or rib, and is formed so as to allow side-to-side movement or floatation of the seal. The trocar seal may further comprise a plurality of radially extending ribs formed on the proximal or distal face of the septum seal to reinforce the seal. The trocar seal may further comprise a tensile element formed on the distal face of the septum seal.

    • A surgical access device includes a single valve that forms a seal with a body wall and provides an access channel into a body cavity. The valve has properties for creating a zero seal in the absence of an instrument as well as an instrument seal for an instrument having a diameter up to about 37 mm. The valve can include a gel material and the access channel can include a protective sleeve to provide for wound protection during insertion and withdrawal of a sharp surgical instrument. The valve further comprises a cap ring which may be inserted or molded with the gel material. The protective sleeve may be bonded or molded around an inner diameter of the cap ring. The protective sleeve may be a single tubular member, or may comprise a plurality of axially extending sleeve members having a plurality of axial slits. The protective sleeve and the cap ring may comprise of the same or different materials. The surgical access device further comprises at least one support ring disposed circumferentially of the valve forming a hollow space, and a wound retractor operatively placed in the hollow space. The wound retractor includes an inner ring, an outer ring, and a flexible sleeve connecting the inner ring and the outer ring.
