    • An apparatus is provided to allow for an adjustable length tether for use in the spine and other parts of the body. The tether comprises an artificial strand with an eyelet formed in one end, the other end being looped through the eyelet. The other end is then secured with respect to the eyelet by a crimp, the excess length being cut off after the length of the tether has been given an appropriate tension. Alternatively, the eyelet end may be formed around a grommet. The crimp may be separate from the grommet or a part of the grommet. The mechanism by which the length is adjusted in some cases will take advantage of the shape memory properties of alloys such as nickel-titanium.

    • Devices are currently implanted in patients to treat pain and other conditions including incontinence by delivering stimulants to the nervous system. Surgical methods are improved for implanting the devices. The soft tissues of the body are dilated above the site of device implantation, for example, above the sacrum, to create an annulus of dilated soft tissues. The tissues are dilated by introducing into the soft tissue sequentially larger cannulated dilating devices. A tubular retractor is then passed over the largest introduced dilating device and at least a number of the dilating devices are retracted. This establishes a tubular device insertion corridor through the dilated soft tissues to the site of implantation, for example, the foramen of the sacrum. The device to be implanted, such as an electrical lead, is then inserted through the corridor. The device is tested in situ. Finally, such other method steps are performed, including closing the corridor, as implantation requires. As preferred, when establishing a corridor through the tissues, a corridor is established sufficient in size for insertion of an endoscope; and the endoscope is inserted and utilized for visual imaging as needed. Also as preferred, the nervous system of the body is tested for appropriate reaction to the stimulant through insertion and manipulation of testing implements through the corridor before performing other method steps. As further preferred, the device to be implanted is anchored through insertion and manipulation of anchoring implements through the corridor. The device is also connected to associated equipment, such as an implantable pulse generator, by tunneling through the soft tissues and passing an extension, a cable, from the lead to the site of implantation of the other equipment, such as in the abdomen.

    • A new top-loading transverse connector of the present invention for bridging between longitudinal members of a spinal fixation system implant. The transverse connector is loaded and clamped to the longitudinal member from the top permitting easy access and adjustment and may clamp at the same location along the rod as a vertebral fixation element. The transverse connector comprises an elongate body and an engagement portion on each opposing end of the elongate body. The engagement portion includes downward legs each with a surface to engage the longitudinal member. The downward legs are displaced from each other to define a space therebetween. The engagement portion includes an upper surface defining an opening therethrough. The opening intersects the space defined between the downward legs. Together, the space and opening are configured to receive the clamp assembly carried on the longitudinal member. Another object of the invention, is to provide an improved system including the transverse connector. Another object of the invention is to provide a transverse connector that clamps to the longitudinal member at a variable angle. Another object of the invention is to provide a separate engagement portion which can be used with existing transverse connectors to permit clamping at the same site as vertebral fixation elements. Another object of the invention is to provide a way to adjust the length of the elongate body of the transverse connector.

    • An attachment plate for use with a spinal rod system converts a normally side-tightening eyebolt to a top-tightenable arrangement for engaging spinal fixation elements to the spinal rod. The plate is configured for use with spinal fixation elements having a posteriorly projecting central post, one lateral surface of the post contacting the spinal rod when the rod extends through an aperture of the eyebolt. The attachment plate is generally L-shaped with a slot in one portion for receiving the posteriorly projecting threaded post of the eyebolt. The plate includes a flange portion having a surface configured to engage the lateral surface of the fixation element post opposite the spinal rod. The attachment plate includes a camming segment to provide a clamping force component directed toward the flange portion of the plate to thereby tightly clamp the spinal rod to the fixation element post when a nut is threaded onto the eyebolt threaded post.

    • An anterior cervical plating system is disclosed which includes an elongated plate having a pair of bone screw bores at opposite ends of the plate, and with bone screw slot configurations in the middle bridge portion of the plate. A single locking screw is provided at each end of the plate which engages the heads of the bone screws trapped within recesses of the plate. The locking screw provides a rigid fixation of the screws to the plate, while the heads of the screws are flush or recessed below the upper surface of the plate. The invention further contemplates a drill and tap guide assembly which can be mounted on the fixation plate to provide a firm foundation for accurately drilling and tapping screw holes into the vertebra to be instrumented. The drill and tap guide assembly includes an assembly support which is engaged to the plate by way of a positioning screw and cross pins mounting the positioning screw to the guide body. A tap sleeve and drill guide can then be supported by the assembly support, which both thereby provide accurate positioning for a drill.
