    • A colposcopic surgical punch instrument includes a hollow tube and a push rod extending through the hollow tube. A handle is coupled to the proximal end of the tube and an actuation lever is pivotally coupled to the handle and the proximal end of the push rod for effecting axial movement of the push rod relative to the tube. A stationary jaw in the form of a die having an open top and bottom is coupled to the distal end of the tube and a movable jaw in the form of a punch is pivotally coupled to the stationary jaw by a tongue and groove arrangement. In one embodiment, the stationary jaw is provided with a pair of ramped projections on its sides and a tab projection on its distal end. A removable plastic sleeve fits over the stationary jaw. The plastic sleeve has an open proximal end and an open top. A pair of holes in the sides of the sleeve engage the ramped projections on the sides of the stationary jaw and a hole in the distal end of the sleeve engages the distal tab on the stationary jaw. The top opening of the sleeve is dimensioned to allow free movement of the movable jaw into and out of the stationary jaw. When attached to the stationary jaw, the sleeve functions as a multiple sample basket beneath the open bottom of the stationary jaw. In another embodiment, the jaw has surface recesses and the sleeve has interior projections.

    • An endoscopic tool for dispensing units of hemostatic agent broadly comprises a hollow tube which holds a plurality of individual units of the hemostatic agent, a valve at the distal end of the hollow tube which permits the hemostatic agent to pass therethrough but prevents foreign matter from contacting the hemostatic agent while in the tube, a plunger which extends into the hollow tube and contacts a proximal unit of the hemostatic agent, and a mechanism for moving the plunger distally in incremental movements to cause, upon each incremental movement, an individual unit of the hemostatic agent to be pushed through the valve. Typically, the mechanism for moving the plunger and the proximal end of the hollow tube are held in a handle. One mechanism for moving the plunger utilizes a portion of the plunger as a ratchet by forming a plurality of conical elements thereon, and utilizes a switch in the handle which has a V-spring coupled thereto as a pawl for advancing the conical elements. A second mechanism utilizes a regularly shaped notched cutout in the handle as the ratchet, and a vertical protrusion on the plunger which extends through the cutout as an engagement element. The notched cutout provides resilient stops, and the protrusion is shaped to allow it to be forced in a forward direction through each resilient stop and then to sit in another notch.

    • A tissue core biopsy cannula of the present invention includes a distal end which is ground in two planes which intersect each other at a first angle along a line which intersects the longitudinal axis of the cannula at a second angle. This produces a cardioid shaped opening in the distal end of the cannula. The entire edge of the sidewall is thereby inclined inward toward the distal opening and two distal cutting edges are provided. According to the presently preferred embodiment, the first angle at which the two planes intersect is approximately 60.degree. and the second angle at which the intersection of the two planes intersects the longitudinal axis of the cannula is approximately 20.degree.. In order to aid in removal of the biopsy sample from the target tissue site, the cannula according to the invention is provided with side fenestration holes and/or a split end which aids in grasping the tissue so that it can be pulled away from the target tissue site. The cannula according to the invention may be used with a solid stylet or a notched stylet and may be provided as part of a manual or automatic biopsy tool. A preferred stylet for use with the cannula has a distal end which is also ground in two planes like the distal end of the cannula.

    • A bipolar roller electrode has a non-conductive base upon which two electrical conductors are placed with a gap between the conductors. The electrode is provided with a blind hole at each end thereof for rotatably mounting it between a pair of arms. Each of the two conductors preferably extends into or forms a part of a respective blind hole. An electrocautery probe according to the invention includes a pair of electrically conductive arms between which the electrode is mounted with each of the arms entering one of the blind holes in the electrode. The arms are mechanically joined but electrically isolated and their proximal ends are individually coupled to a pair of electrode leads. A non-conductive or insulated mounting sleeve is preferably provided intermediate of the arms and the leads for slideably coupling the probe to a resectoscope. The distal end of each arm of the probe enters a respective blind hole in the roller electrode and makes electrical contact with a respective one of the two conductors. Several methods of manufacturing the electrodes are disclosed as are several different embodiments of the electrodes.

    • A probe for use with an endoscopic suction-irrigation instrument is disclosed which permits a second probe to be inserted therethrough. The suction-irrigation instrument has a cannula, a fluid chamber having at least one port for irrigation and suction and a proximal opening, and a first seal in the proximal end of the fluid chamber. The fluid chamber has a distal end communicating with the cannula. The probe has a proximal handle portion, a middle portion, and a hollow distal portion. The handle portion has an axial passageway and a second seal therein. The middle portion extends from the handle portion through the first seal at the end of the fluid chamber and through the fluid chamber. The middle portion has an outer diameter shaped to contact the first seal such that the proximal end of the fluid chamber is sealed. The middle portion is also shaped to be smaller than the inner diameter of the fluid chamber, and the middle portion is provided with a second axial internal passageway extending therethrough. The distal portion extends at least partially through the cannula and has an outer diameter smaller than the inner diameter of the cannula. A second probe may be inserted through the second seal of the first probe, through the axial passageway in the handle portion, through the internal passageway in the middle portion, and out through the distal portion and the cannula.
