    • In accordance with one embodiment of the present invention, an osteodistraction device includes a flexible rod having a threaded portion. A distraction arm that includes a rod guide is coupled to the threaded portion of the flexible rod through a threaded hole near one end of the distraction arm. The rod guide of the distraction arm is operable to slide through a pivoting guide that is coupled to the distraction arm. An adjustment screw is coupled to the pivoting guide and is operable to rotate the distraction arm from a first to a second orientation. The flexible rod is operable to change the position of the rod guide relative to the pivoting guide.

    • The present invention provides an improved orthopaedic system for the modification of the distance between the contralateral maxillary segments or halves. In a preferred embodiment, the system includes two metal bone-plates fixed on the bony palatal shelves, with slotted extensions which opposite internal walls have concave indents into which disk-shaped wings of interchangeable distraction modules fit. The in the palatal plane concave female indent and the convex male wing prevent distraction module dislocation in case of oblique placement in the axial plane. In the frontal plane, the male wings are straight. The extension on the abutment plate has a slit like opening at the caudal side, to be fitted by a suture or a wire of any kind material, passing also in an opening in the male wing of the distraction module, to allow for assembling the units before fixation in the bony palatal shelve. The abutment plate is made partly malleable for an improved accommodation to the local alveolar bony anatomy. The opposing ends of the abutment plate have a shape allowing the anterior side to slip under the mucoperiosteum similar to a dissecting instrument, and the posterior end rounded and blunt in a way to protect the palatal arterioles. The abutment plates are provided by a slotted hole or slotted holes to accommodate the osteosynthesis screws or tacks in a preferred position related to the adjacent dental roots.

    • The present invention provides an improved orthopedic system for the modification of the distance between the maxilla and zygoma. In a preferred embodiment, the system includes first and second footplates attached to an orthopedic device. The second footplate is attached to the zygoma, with the first footplate being mechanically coupled to the maxilla. This mechanical coupling is achieved either through attachment directly to the maxilla or by attachment to a construct wired to the patient's teeth. The orthopedic device, which may be a distractor, allows for modification of the distance between the maxilla and zygoma. The entire system can advantageously be placed intra-orally within a patient. In a preferred embodiment, the device does not increase in length upon activation. In another preferred embodiment, the second footplate is offset, allowing the actuator to be placed under the zygoma. Methods for using this novel orthopedic system are also disclosed.

    • An implantable bone distractor is created which grips both sides of a cut bone by the use of screws and plates, then applies an expansion force between the cut bone edges. This force is applied in order to encourage bone growth in the gap formed between the cut bone edges as they are distracted apart. This force is applied with hydraulic jacks, which are actuated using tubes passing through the skin, allowing the controlled application of force between the cut bone edges from external to the body. The distraction may occur in three-dimensions, with each bone segment being distracted within its own plane. The distraction may be further improved by providing substantially continuous distraction of the bone segments over time, allowing for substantially continuous bone growth.

    • A combination dental implant/prosthetic support and distractor for facilitating distraction osteogenesis comprises a basically cylindrical shaft having a set of threads on the outside surface and a smooth, distal tip which bears against the bone of the jaw of the patient. Rotation of the device in the mouth of the patient, after a suitable surgical cut is made, causes the adjacent tissue to move or ride up the screw threads to create space between the cut jaw segment and the balance of the jaw tissue. This space is then filled in by distraction osteogenesis. The implant/distractor is left in-situ, after the distraction is finished, for securement thereto of a dental prosthetic, either a bridge or crown. In an embodiment of the invention, a flat disc is placed in the blind bore drilled into the jaw such that the distal tip of the shaft of the combined implant/distractor, will bear against it. A protective cap and/or collar can be provided, suitable shaped, to protect the shaft/gum line from bacterial attack. Also, the shaft may be provided with an internal passageway and one or more radially extending branches. Growth-enhancing material can be passed through the passageway and branches to promote bone development and recovery. In another embodiment, a coronal member is provided, extra-osseously, to facilitate the distraction osteogenesis process. In one version of the device, fluting is provided to the exterior surface of the shaft so that bone growth is enhanced at that intersection. Also, the shaft can be provided with a flat disc at its head to reduce rocking of the device in the mouth.
