    • A method for performing spinal fixation and instrumentation. A first incision may be made through the skin and a passageway may be created to the spine. A screw may be inserted through the passageway and into a vertebrae. The screw may have a head portion including a channel. An insertion guide may be operable connected to the screw. The insertion guide may have first and second longitudinal slots. Additional screws may each be inserted through separate incisions or through the first incision. Insertion guides may be operably connected to a head portion of each screw. A sleeve may be positioned into one insertion guide in a first position to guide a rod through at least one other insertion guide. The sleeve may be rotated to a second position to allow the rod to move down the slots of the insertion guide(s) and into the head portion of the screw.

The device also includes at least one bushing insertable in the through hole. The bushing has top and bottom surfaces, a longitudinal axis, a central bore designed to receive a bone screw, and a peripheral outside face having at least two outwardly extending protrusions defining an axis of rotation of the bushing extending through the protrusions. A cross section of the bushing orthogonal to the longitudinal axis is shaped such that the bushing is pivotable about the axis of rotation defined by the protrusions.

    • A bone plate includes an upper surface, a bone contacting surface, and at least one hole extending through the upper and bone contacting surfaces for receiving a bone screw. The at least one hole includes a first portion and a second portion that overlap one another. The first portion defines a substantially circular outer periphery and the second portion defines an elongated outer periphery that is elongated in a direction substantially parallel to the longitudinal axis of the plate. The first portion may have threads configured to engage threads on the head of a bone screw, and the second portion may be configured to cooperate with the head of a different bone screw to provide compression to the fracture.
