    • A valvulotome for disrupting valve cusps in the vein of a patient includes a shaft (14) and a cutting member (12) which extends along an axis that defines an axial direction as well as an angular direction. First and second portions of the cutting member define respective cutting edges which extend outwardly of the axis in an angular relationship and an axial relationship with each other. The first and second cutting edges engage and disrupt different cusps of the valve when the shaft is withdrawn axially from the vein. Each of the cutting edges is protected by a proximate shoulder (123) which inhibits the engagement of side branches by the associated cutting edge. The cutting member may be formed as a helical blade, a wire or a cylinder and may be provided with a swivel coupling (158) as well as a sheath (131) operable to cover or expose the cutting member.

    • A tissue excision catheter system is described, e.g. an atherectomy catheter, that is capable of creating an opening through obstructing tissue in a blood vessel that is greater than the outside diameter of the catheter. The system comprises the combination of:

      (i) a catheter sub-assembly (10) carrying at its distal end a cutting head (30) that can be inserted through the plaque and then retracted so as to excise the obstructing tissue by means of a rearwardly directed cutting edge (32a), and
      (ii) and inflatable cuff (22) that can be placed around the limb and inflated so as to compress the blood vessel at the site of the obstruction during the excision of the obstructing tissue. Following that excision the pressure is released so that the blood vessel expands back to its normal diameter, thereby creating a passageway through the tissue that is greater than the diameter of the cutting edge. Preferably the catheter sub-assembly comprises a central member (45) that is stationary relative to the cutting head and which determines the extend of collapse of the blood vessel under the applied pressure thereby to determine the precise thickness of the layer of tissue removed by virtue of the radial distance of the cutting edge (32a) from the stationary member (45).
