摘要： A surgical apparatus includes a seal anchor member. The seal anchor member includes a first end, a second end, at least one longitudinal port extending between the two ends, and a seal assembly disposed within the at least one longitudinal port. The seal assembly can be positioned at any location along the length of the at least one longitudinal port. The seal assembly comprises a seal housing which is dimensioned to receive a surgical instrument inserted into the at least one longitudinal port and form a substantially fluid-tight seal therewith. The seal housing may comprise a pivotable member to pivot the surgical instrument inserted therein. The at least one longitudinal port may define two gradually enlarging openings to facilitate the maneuverability of the surgical instrument inserted therein. The at least one longitudinal port may comprise a protective sleeve to protect it from accidental penetration by the surgical instrument.

摘要： A surgical apparatus for introduction of laparoscopic instruments into an anatomical cavity through tissue at an entry site. The apparatus includes a body with a frustoconical-shaped wall. The body defines an interior cavity, an open bottom, and a substantially closed top wall with openings from which a plurality of ports extend upward therefrom. The ports are adapted to receive the laparoscopic instruments for introduction through the interior cavity and open bottom of the body into the anatomical cavity. In the preferred embodiment, the frustoconical- shaped wall of the body is placed through an incision in the umbilicus. In one aspect of the invention, the body is a unitary one-piece molded structure. A reinforcing belt or plate formed from a relatively hard material can be integral to the body, and separately formed port caps each having a septum may be bonded to the ports.

摘要： Methods and devices are provided to improve access to a surgical site during surgical procedures. In one exemplary embodiment a surgical access device (10) is provided having a cannula (60), a cannula base (40), and a retractor (20). The cannula base (40) is configured to couple both to the retractor (20) and the cannula (60) to provide access to a surgical site. The cannula (60) can be removably and replaceably coupled to the cannula base (40). Optionally, the cannula base (40) can be removably and replaceably coupled to the retractor (20). As desired, the cannula (60) can be decoupled from the cannula base (40), which can allow for an object to be removed from the surgical site and/or for the cannula (60), or another cannula to be attached to the cannula base (40) that remains in place to maintain access to the surgical site. Exemplary methods for accessing a surgical site are also provided.
