摘要： Medical devices, systems, and methods reduce the distance between two points in tissue, often for treatment of congestive heart failure and often in a minimally invasive manner. An anchor is inserted along an insertion path through a first wall of the heart. An arm of the anchor is deployed and rotationally positioned according to a desired alignment. Application of tension to the anchor may draw the first and second walls of the heart into contact along a desired contour so as to effect a desired change in the geometry of the heart. Additional anchors may be inserted and aligned with the first anchor to close off a portion of a ventricle such that the ventricle is geometrically remodeled and disease progression is reversed, halted, and/or slowed.

The elongate shaft comprises an anastomotic ring deployment mechanism (20) on a distal portion of the shaft. The ring deployment mechanism is moveable from an unactuated position to an actuated position. The elongate shaft comprises a rigid, curved member, thereby rendering the tool applicable for colorectal applications. In one embodiment, the tool comprises an actuating mechanism (22,24) that is operable to transfer an actuating force to the ring deployment mechanism via a flexible connecting member (54,56).

The distal portion comprises a ring deployment mechanism (26), which is configured to receive and deploy an anastomotic ring (14). The proximal portion comprises a mechanism that is operable to remotely actuate the ring deployment mechanism. The distal portion is removable from the proximal portion. The distal portion may be selectively engaged with the proximal portion by a series of annular flanges (114) and clamp arms (110) that are configured to engage the annular flanges. The ability to remove the distal portion of the instrument permits the replacement of the ring deployment mechanism without requiring the replacement of the instrument in its entirety.

The shaft includes conduits (96) for communicating a pressurized medium to the inflatable members and an insufflation tube. The handle includes a port for communicating a pressurized medium to the conduits and a port for communicating a pressurized medium to the insufflation tube. After an anastomotic ring (14) has been deployed, the shaft may be positioned such that an inflatable member is positioned within each of the lumens joined by the anastomosis, such that an inflatable member is located on each side of the anastomosis. The inflatable members may be inflated to provide a seal in each of the lumens. With each of the lumens sealed by the inflatable members, the anastomosis site may be insufflated through the insufflation tube to leak or pressure test the anastomosis.

摘要： A surgical instrument (30) comprises a handle (32) and an elongate shaft (34), and is operable to remove an anastomotic ring device from a patient. The elongate shaft houses a plurality of ring engaging fingers (38). Each of the fingers comprises sheath (40) housing a wire (46) having a hook (48) at its distal end. Each hook may be advanced beyond the distal end of a respective sheath to engage petals on an anastomotic ring device. Subsequent proximal retraction of the hooks secures the anastomotic ring device to the ring engaging fingers. With the anastomotic ring device secured to the ring engaging fingers, the fingers may be drawn together to collapse the anastomotic ring device from an actuated, hollow rivet-shaped configuration to an unactuated, cylindrically-shaped configuration. The surgical instrument may then contain the collapsed anastomotic ring device, thereby facilitating removal of the anastomotic ring device from the patient.
