摘要： A surgical stapling device comprises a shaft 10, a surgical staple 14 slidable longitudinally within the shaft, and a staple-firing mechanism 16, 24 for driving the staple towards the forward end of the housing, bending the staple to bring the free ends of the legs towards one another to close the staple, and releasing the closed staple. The device also includes a blood locator tube 18 slidable longitudinally within the shaft between an initial extended position wherein a forward end of the tube projects beyond the forward end of the shaft to enter a puncture site in a liquid- carrying vessel in a human or animal, thereby to locate the forward end of the shaft at the puncture site, and a retracted position wherein the tube is retracted into the housing in coordination with the closure of the staple. The tube 18 has a longitudinal slot 20 extending rearwardly from its forward end and the back of the staple is accommodated transversely in the slot.

摘要： An implantable marking device is provided which is designed to percutaneously deliver permanent markers to desired tissue locations within a patient's body, even if the desired locations are laterally disposed relative to the distal end of the delivery device, as is the case for conduit or cavity walls. This provides several advantages to the physician in diagnosis and management of tissue abnormalities, such as a means of localization of a tissue abnormality for follow-up surgical treatment, and a means of tissue abnormality site identification for purposes of ongoing diagnostic follow-up.
In one preferred construction, a radiographic clip is configured in the form of a surgical staple. A disposable tissue marker applier, which comprises a flexible tube, pull wire, and squeeze handle, is employed to advance and deploy the clip to a desired tissue location. Either a flexible or a rigid introducer is also provided for providing access to the site to be marked.

摘要： A clip applier (10) for applying a surgical clip (26) in a patient comprising a disposable cartridge (12) and a reusable actuating mechanism (14). The cartridge includes an elongate tube (12) having a proximal end and a distal end, a pair of opposing jaw members (34) extending outwardly from the distal end of the elongate tube, and a one-way ratchet mechanism providing a full actuating stroke. The actuating mechanism is coupled to the proximal end of the elongate tube to move the jaw members between an open position and a closed position. The ratchet mechanism may be formed from injection molded plastic. The cartridge further comprises a push member (24) for advancing the clip into the jaw members, a biasing spring (22) for maintaining the push member against the clip, and a drive coupling (38) operatively connected to a sliding ratchet pawl for engaging with mating teeth formed on an inner surface of the elongate tube, wherein the ratchet pawl (4) includes a cantilever arm (46) that engages with the mating teeth (44), and wherein the drive coupling provides a central connection of the ratchet pawl allowing pivoting and improved seating of the mating teeth.

摘要： A method and apparatus for closing a vascular wound includes a guidewire and/or other surgical implement extending from the wound. A hemostatic material is advanced over the surgical implement and into contact with an area of the blood vessel surrounding the wound. The surgical implement is removed. Blood soaks the hemostatic material, and blood clotting is facilitated by the hemostatic agent within the material. A sealing layer of adhesive can be applied to the hemostatic material, confining the blood flow to the material. Thus, the vascular puncture wound is sealed by natural blood clot formation.

The ligation clip includes a hub portion (16,18) and ligation arms (24, 26) for occluding a vessel. Ligation arms are independently rotatable about a common axis that is parallel to the longitudinal axis of the ligation arms. The ligation arms are biased in a first position by a biasing mechanism. The ligation arms of the clip are rotatable through at least one of a plurality of positions wherein it has a larger diameter than an access device. An applier receives at least one ligating clip in a first position and delivers the ligating clip to a vessel in a second position. Occlusion of the vessel occurs when one of the ligation arms is rotated into a third position.

The prostheses may be self-expanding or balloon expandable. After initial placement, a fastener applier system (27) is introduced within the expanded prostheses to deploy a plurality of fasteners at at least one prosthesis end, usually as each end of the prosthesis. The fasteners are usually helical fasteners which are delivered from a helical track in the fastener applier by rotation with a rotator wire. The fasteners will be applied singly, typically in circumferentially spaced-apart patterns about the interior of each end of the prosthesis.

摘要： The present invention relates to devices and methods for creating and securing a tissue fold during an endoluminal medical procedure. The devices and methods may be used for folding and securing, for example, a fundus wall onto an esophagus wall. An aspect of the invention includes a two-piece tissue clip configured to be installed through an endoluminal device to secure a tissue fold. The clip includes a female member and a male member configured to engage one another to secure the tissue fold. Another aspect of the invention includes a clipping device comprising a tissue clip magazine. The magazine is configured to hold a plurality of tissue clips and install the plurality of tissue clips in a single actuation of the magazine.
