摘要： PURPOSE: To prevent a bad side reaction such as surgical deuteropathly, by mounting a soft insulator on one side of a soft conductive mesh to be conducted from a soft conductive wire and providing the soft insulator with a surface expanding beyond the periphery of the soft conductive mesh. CONSTITUTION: A defibrillation electrode 36 is attached to a silicone backing material 40 and formed of a platinum mesh 38. The diameter of the platinum mesh 38 is set to be smaller enough than the diameter of the silicon backing material 40 and positioned substantially at the center of the silicone backing material 40. With this, a circumferential area 54 for attachment to a cardiac vesicle is formed by using a staple. This silicone backing material 40 is adhered to the platinum mesh 38 with an adhesive of Silastic brand to make it function as an electrical insulator. A conductive wire 42 wrapped with a silicone insulator 44 is attached to the platinum mesh 38.

摘要： PURPOSE: To restrain the risk of external wounds to a surgical operation and the heart to the minimum by getting access to the interior of the pericardium through a slot in the lower edge of the pericardium and a tunnel cut open between the pericardium and the diaphragm. CONSTITUTION: A breast work is formed with a generally xiphoid excised part 42, and the lower edge of the pericardium P is sandwiched by a clamp 44 through the xiphoid excised part. An access device 12 to the pericardium is inserted in the excised part 42, and a lower jaw portion 16 is inserted in the tunnel T, and an upper jaw portion 14 is inserted in the pericardium through the slot 46 respectively. Both upper and lower jaw portions of the access device are connectively crossed to grasp the anatomy of the pericardium, insert a guide wire into the pericardium and extend the same through the lower pericardium. The access device 12 in the pericardium is removed, the guide wire 50 is left to get access to the inside of the pericardium and electrodes E1 , E2 for a fine movement remover are inserted in a vagina between the pericardiums.

摘要： PURPOSE: To make it possible to shear arteriosclerotic deposits from the lesion artery by providing the device with a gripper which is freely selectively freely dilatable in a radial direction after insertion in a shrunk state into the artery. CONSTITUTION: A cut 40 is formed near the arteriosclerotic deposits 6. A shearing member 18 is put into the radially shrunk state and its terminal is introduced into this cut and the device is inserted. While the device is introduced during the insertion, a handle 10 is rotated around an axial line to decrease the torque of the device acting on the artery 5. The shearing member 18 is dilated in the radial direction after crossing the required length of the deposits 6 and is securely pressed to the deposits 6 to be removed and is engaged therewith. The device is removed after this engagement and the part of the previously crossed deposits 6 is sheared away. The pulling is executed by pulling a cup 15 so as to engage the cup with the grip 12, by which the exertion of torsion force on the device while the grip is rotated in the cup is averted as the device is pulled.

An inflatable catheter 64 for eliminating an occlusive material is passed through a thrombus 58 by going out from the end portion 14 through the sleeve 10. The inflatable catheter 64 for eliminating the occlusive material is extruded together with a balloon 66 of a contracted state. The balloon 64 is inflated and the thrombus 58 is entered into the end portion 14 of the sleeve 10. The catheter 64 is drawn out from the sleeve 10, a thrombocyte separator 68 is inserted into the sleeve 10 and the thrombus 58 is smashed so as to eliminate it.

摘要： PURPOSE: To obtain a clear endoscopic image by including an irrigation port for centering a blood vessel mirror and injecting into vessels during observation of the vessel and carrying out observation with keeping blood steam. CONSTITUTION: Once drawing an inner catheter 12 is pulled in a proximal end direction with distal ends of a sheathing 14 and the inner catheter 12 being brought into contact with the sheathing 14, a longitudinal slit group 16 on the sheathing enlarges in a radial symmetrical form around an axis of a centering catheter 10. By such operation, the centering catheter 10 is separated from a vessel wall and performs the centering of a vessel mirror 18 during observation of a curved position in the vessel. Additionally, blood stream continues without blockage even if the slit group 16 is expanded. Irradiation by the catheter 10 for centering the vessel mirror generally uses physiological saline solution. The catheter is connected via an irrigation port 20 and an irrigation duct 28 to a pressure device of the solution. By pouring a certain specified washing liquid via the inner catheter 12 in the blood vessel according to instruction of a computer, an object can be observed clearly from a tip of the vessel mirror 18.
