    • PURPOSE: To restrain the risk of external wounds to a surgical operation and the heart to the minimum by getting access to the interior of the pericardium through a slot in the lower edge of the pericardium and a tunnel cut open between the pericardium and the diaphragm. CONSTITUTION: A breast work is formed with a generally xiphoid excised part 42, and the lower edge of the pericardium P is sandwiched by a clamp 44 through the xiphoid excised part. A tunnel T is cut out toward the rear surface of the pericardium between the pericardium P and the diaphragm D. While the lower edge of the pericardium P is sandwiched by the clamp 44, a small slot 46 is excised in the lower edge of the pericardium P by the use of a scalpel 48. An access device 12 to the pericardium is inserted in the excised part 42, and a lower jaw portion 16 is inserted in the tunnel T, and an upper jaw portion 14 is inserted in the pericardium through the slot 46 respectively. Both upper and lower jaw portions of the access device are connectively crossed to grasp the anatomy of the pericardium, insert a guide wire into the pericardium and extend the same through the lower pericardium. The access device 12 in the pericardium is removed, the guide wire 50 is left to get access to the inside of the pericardium and electrodes E1 , E2 for a fine movement remover are inserted in a vagina between the pericardiums.

    • PURPOSE: To obtain a clear endoscopic image by including an irrigation port for centering a blood vessel mirror and injecting into vessels during observation of the vessel and carrying out observation with keeping blood steam. CONSTITUTION: Once drawing an inner catheter 12 is pulled in a proximal end direction with distal ends of a sheathing 14 and the inner catheter 12 being brought into contact with the sheathing 14, a longitudinal slit group 16 on the sheathing enlarges in a radial symmetrical form around an axis of a centering catheter 10. By such operation, the centering catheter 10 is separated from a vessel wall and performs the centering of a vessel mirror 18 during observation of a curved position in the vessel. Additionally, blood stream continues without blockage even if the slit group 16 is expanded. Irradiation by the catheter 10 for centering the vessel mirror generally uses physiological saline solution. The catheter is connected via an irrigation port 20 and an irrigation duct 28 to a pressure device of the solution. By pouring a certain specified washing liquid via the inner catheter 12 in the blood vessel according to instruction of a computer, an object can be observed clearly from a tip of the vessel mirror 18.
