摘要： Prosthetic nucleus apparatus (10) and methods for treating an intervertebral disc are disclosed. Prosthetic nucleus apparatus (10) may include a barrier sealant membrane (12) and a prosthetic nucleus material (14). The barrier sealant membrane (12) forms a chamber (24) which can receive the prosthetic nucleus material (14). The barrier sealant membrane (12) can be formed by depositing a layer of material on a tissue surface (102) within a de-nucleated space (104) within an intervertebral disc (100). The prosthetic nucleus material (14) may be positioned within the chamber (24) of the barrier sealant membrane (12) after the barrier sealant membrane (12) is deposited within the de-nucleated space (104). The barrier sealant membrane (12) and the prosthetic nucleus material may be positioned within a patient through an axial trans-sacral bore (410). A plug 16 may also be included to prevent expulsion of the barrier sealant membrane (12) and prosthetic nucleus material (14).

摘要： Cutter assemblies for use with cutter blades made of shape memory materials. The cutter blades may be deployed in the interior of an intervertebral disc space and rotated relative to a central axis of the cutter assembly to cut the material present for removal from the intervertebral disc space. Cutter blades with different attributes (such as throw length, cutter blade angle, type and location of blade edges) are adapted to achieve different objectives within the intervertebral disc space. Some cutter blades are adapted to promote bleeding of cartilage and vertebral body endplates and some cutter blades are adopted to avoid causing such bleeding. Closed loop cutter blades are described which have certain desirable attributes including the ability to remove the entire cutter blade from the intervertebral disc space after a break in the blade. Serration patterns are disclosed including a serration pattern that makes use of trapezoidal serrations.

摘要： A spinal mobility preservation apparatus and methods are disclosed. The spinal mobility preservation apparatus (10) may include a proximal body (12), an intermediate body (14), a distal body (16), and an expandable membrane (18). The proximal body (12) and the distal body (16) secure the mobility preservation apparatus (10) to adjacent vertebral bodies (800, 900). At least one of an intermediate body (14) and an expandable membrane (18) secure the proximal body (12) to the distal body (16) and provide a degree of support to a spinal motion segment defined by the adjacent vertebral bodies (800, 900). A single proximal body (12) and an expandable membrane (18) may also compose a spinal mobility preservation apparatus (10). The proximal body (12) secured to one of a superior or an inferior vertebral body and the expandable membrane (18) extending into the intervertebral disc space (860) to support the spinal motion segment.

摘要： Apparatus for providing percutaneous access to the human sacral and lumbar vertebrae in alignment with a visualized, trans-sacral axial instrumentation/fusion (TASIF) line in a minimally invasive, low trauma, manner. A number of related TASIF methods and surgical tool sets are provided by the present invention that are employed to form a percutaneous pathway from an anterior or posterior skin incision to a respective anterior or posterior target point of a sacral surface. The percutaneous pathway is generally axially aligned with an anterior axial instrumentation fusion line (AAIFL) or a posterior axial instrumentation/fusion line (PAIFL) extending from the respective anterior or posterior target point through at least one sacral vertebral body and one or more lumbar vertebral body in the cephalad direction and visualized by radiographic or fluoroscopic equipment. The anterior or posterior percutaneous pathway so formed enables introduction of further tools and instruments for boring one or more respective anterior or posterior TASIF bores in the cephalad direction through the one or more vertebral bodies and intervertebral spinal discs, if present. A single anterior or posterior TASIF bores is preferably aligned axially with the respective visualized AAIFL or PAIFL, and plural anterior or posterior TASIF bore are preferably aligned in parallel with the respective visualized AAIFL or PAIFL. Introduction of spinal implants and instruments is enabled by the provision of the percutaneous pathway in accordance with the present invention and formation of the anterior or posterior TASIF bore(s).

摘要： A system or assembly for anchoring percutaneous devices, e.g., tubes or catheters or leads, extending percutaneously through a percutaneous passageway from the skin of a patient to a subcutaneous location or body cavity or tract or lumen from movement further into the body or retraction out of the body. An implantable anchor (14) comprising an anchor body (16) and a tensioning filament (20) is inserted through the percutaneous passageway and deployed subcutaneously with the anchor body in engagement with body tissue in a manner that inhibits it from being retracted through the passageway. The tensioning filament (20) extends from the anchor body and alongside the percutaneous device body through the percutaneous passageway and tension is applied to it to draw the deployed anchor against subcutaneous body tissue and toward the patients skin. The tensioning filament is coupled with an attachment mechanism (50) secured against the percutaneous device body outside the patient's skin at a location adjacent to the patient's skin. The assembly of the deployed anchor within the patient's body with the external attachment mechanism inhibits advancement or retraction of the percutaneous device body through the percutaneous passageway.
