    • PROBLEM TO BE SOLVED: To provide an activating mechanism for an endoscopic ligator, mounted on an endoscopes of every form and accurately controlled to perform ligature.
      SOLUTION: This activating mechanism for activating the endoscopic ligator 14 includes: a mounting element; and an activating element connected to the mounting element, wherein the mounting element has such dimensions that it is directly inserted in an opening part of the base end of an operating channel 16a of an endoscope 10a and fitted in the working passage to mount the activating mechanism on the endoscope and includes a passage for receiving an activating braid of the ligator, which is introduced into an activating element of the activating mechanism through the working passage and a passage of the mounting element. The activating braid 15 provides a driving pin and a roller clutch mounted on a spool of the activating element to set the activating element in an engagement position or disengagement position and a knob for rotating the driving pin. When the driving pin engages with the roller clutch, the knob can be rotated only in one direction, and when it is in the disengagement position, the knob can be rotated in both directions.

    • PURPOSE: To obtain the medical cutting tool of sphincter for cutting a patient's sphincter along a cutting surface under the control of resisting to the externally bending of the cutting surface by making the second moment of inertia in the cross section larger than the first one. CONSTITUTION: A lead wire 43 is arranged in the first vertical inner cavity 55, and an intensified wire material 45 is preferably made of a spring steel, and is arranged in the second vertical inner cavity 57, preferably along the whole length of the inner cavity 57. The inner cavity 57 and the intensified wire material are preferably formed rectangular, and the width (W) is larger than the height (H). When a controlling handle 29 is operated, a blade tip 53 is moved within the cut surface and the patient's sphincter is cut. This movement is caused by stretching the lead wire 43 of pulling a cutter 33 from an opening, and by pulling a remote position 47 to a near direction and by making the blade tip 53 to sweep out the cut surface. The bending is carried out around a bending axis (B), in contrast the bending receives a resistance around a resistant axis (R). Since the width (W) is larger than the height (H), the moment of inertia in the cross section (IR) is larger than the same (IB). Accordingly, there is a larger bending resistance around an axis (R) than around an axis (B).
