    • PROBLEM TO BE SOLVED: To provide a template assembly to mark an adhesion device transplanting position through the smallest incision part. SOLUTION: A template assembly 15 is pivotally moved so that a position for the transplantation of an in vivo adhesion device or the introduction of a work instrument is marked on an intervertebral disk annulus. The template assembly 15 is introduced by stinging into the body, and has a tubular main body 17 having a dimension such as being advanced into a damaged intervertebral disk. A long and narrow shaft 19 engages by a screw with the main body on the base end by slidably extending on the inside of the tubular main body 17, and advances the shaft 19 through the main body by rotation of the shaft 19 related to the main body. A guide foot 21 is pivotally fixed to the tail end of the tubular main body 17, and is pivotally moved to a second unfolding position substantially vertically pointed to the tubular main body 17 from a first position fitting to the tubular main body 17 by advancing the shaft 19 through the tubular main body 17. The guide foot 21 has a bore, and a work tip of an electric cautery extends through this bore, and forms a mark on an intervertebral disk annulus.

    • A spinal implant system (20) for correcting spinal deformities and abnormalities includes an elongated spinal rod (21) spanning across several vertebral levels and supporting a number of fixation elements, such as spinal hooks (25). At least some of the fixation elements include an elongated stem (26) projecting from a vertebra engaging portion, connected to the spinal rod by way of a rod connector (28) having a one-piece body defining a rod channel (47) adapted to receive the spinal rod therethrough, a stem bore (49) adapted to receive the stem therethrough, and a threaded bore (54). The stem bore and rod channel intersect to permit contact between the rod and the stem of a fixation element and the threaded bore intersects either the rod channel or the stem bore so that a set screw (58) threaded into the threaded bore bears against either the spinal rod or fixation element stem. The rod channel includes a channel opening (48) to permit top-loading of the rod connector.

    • The eye and threaded shaft of an eyebolt (46) are formed by a combination of two shell-like portions (46A, 46B), each having threaded bosses (48A, 48B) at distal ends thereof. The distal ends are placed around a spinal rod (14) simultaneously with insertion of one of the shell-like portions through the yoke (27, 28) of a spinal hook (22). Then the bosses are brought toward each other and the two bosses combine to form a bolt onto which a nut (36) is threaded to clamp the eyebolt, and thereby the hook, tightly on a spinal rod. The two threaded bosses have an interfitting rib (51) and groove (49) to maintain alignment of the threads of the two bosses. The eyebolt of this invention allows a connecting hook to be installed on a spinal rod in situ between other devices already installed on the spinal rod.
