    • An implantable adjustable sphincter system is comprised of a band configured to encircle a portion of an anatomical passageway, a manual pump, a reservoir in fluid communication with the pump, and a valve assembly in fluid communication with the band and the manual pump. The valve assembly is comprised of a first configuration and a second configuration, and is operable to be manually switched between configurations. The first configuration only permits fluid to flow from the band toward the reservoir. The second configuration only permits fluid to flow from the reservoir toward the band. The manual pump is in fluid communication with the valve assembly and the reservoir, and is manually operable to transfer fluid between the reservoir and the band when the valve assembly is in the second configuration.

    • In accordance with the present invention, there is provided a surgical staple having first undeployed shape for loading into a stapler, and a second deployed shape for connecting tissue together. The staple has a crown and first and second legs, one attached to each end of the crown. The legs extend from the crown in a direction substantially perpendicular to the longitudinal axis of the crown when the staple is in its first shape. The legs comprise first and second layers of material joined together. The first layer of material is a superelastic alloy having a relaxed configuration substantially in the staple's second shape. The second layer of material is a linear elastic material having a relaxed configuration substantially in the staple's first shape. The second layer of material has sufficient rigidity to keep the first layer in the first shape prior to the staple being deployed.

    • A needle or cylindrical element position lock includes a disposable template assembly having front and back templates halves which define an array of close-fitting through-holes to guide needles therethrough. A series of lock slides locks the positions of columns of needles positioned within the template. Each lock slide locks the position of one or more needles inserted into one or more series of the through- holes in the template, with a preferred embodiment locking two series of needles. Each lock slide has the capability of locking one or more series of needles inserted into through-holes in the needle guide template by an axial motion applied by a lock lever or cam. When the lock slide is moved axially, a locking function is provided by a wedge/clamp which engages an inserted needle. For a typical template arrangement, six lock slides are used to lock twelve columns of needles inserted into through-holes in the template assembly.

    • An improved surgical stapling instrument is provided for applying surgical staples to human tissue. The instrument has an articulated stapling head assembly mounted by a rotatable and flexible support shaft on an actuator handle assembly with an improved actuator mechanism for closing and firing the stapling head assembly. The stapling head assembly comprises a fixed jaw with a staple cartridge and a movable jaw with an anvil for clamping the tissue therebetween. The actuator mechanism comprises a jaw closure lever for closing the jaws and a staple firing lever for actuating a staple driver to drive the staples from the staple cartridge into the tissue and against the anvil. The actuator mechanism also includes a cam pulley mounted for rotation by the staple firing lever and secured to a firing cable for actuating the staple driver. A set of drive gears rotatably positioned on opposite sides of the cam pulley mesh with gear sectors on the staple firing lever to rotate the cam pulley and apply tension to the firing cable when the staple firing lever is actuated. The cam pulley has a contoured cam lobe for actuating the firing cable with different mechanical advantages as the pulley is rotated. An anti-backup member engages the cam pulley to prevent the pay-out of the firing cable before and after the staple firing lever is actuated. The length of the support shaft can be adjusted to set the tension in the closure cable so that the jaws are completely closed by the jaw closure lever to produce staples of uniform height when the staple firing lever is actuated.
