    • A cordless cautery and cutting surgical device includes a modular battery and a surgical handle. The battery has a cordless radio-frequency-signal-generation assembly generating an output radio-frequency signal and a first selectively removable connector part. The surgical handle has a first handle body portion with a bipolar cautery and cutting end effector. The end effector has jaws with bipolar contacts and a cutting blade disposed between the jaws. The handle has a second body portion connected to the first body portion, defines therein an aseptically sealable battery-holding compartment selectively exposed to the environment and removably holding therein the battery, and a second selectively removable connector part operable to removably hold the first connector part thereto. The second connector part has conductors electrically connecting the RF assembly to the bipolar contacts for supplying the radio-frequency signal to the bipolar contacts when the first and second connector parts are removably secured together.

    • A multifilar flexible rotary shaft includes a plurality of individual filaments which are not wound around each other or around a central core, a loose ensemble of filaments. The input ends of each filament are coupled to each other and the output ends of each filament are coupled to each other. Preferably all of the filaments are identical. A loose ensemble of N filaments can transmit N times the torque of a single filament, and will have N times the torsional stiffness of a single filament, while retaining the minimum radius of operation of a single filament. Since a loose ensemble of filaments does not have any appreciable contact forces among the filaments (because they are not forcibly twisted together), there is no appreciable internal friction or hysteresis. In order to appreciably eliminate all the hysteresis in such an ensemble of filaments, the filaments should be no more than loosely twisted together, if at all. The only reason for loosely twisting the wires together is to allow the ensemble to be easily handled and to insure that the individual wire filaments follow the same general curved path from one end to the other. Several practical applications of the invention are also disclosed.

    • A filter basket device includes two shaft wires, a filter basket adapted to capture emboli and thrombi, and an actuation handle. The shaft wires are loosely twisted together so that they remain adjacent to each other, and yet are able to slide relative to each other with little friction and without plastic deformation. One shaft wire is connected to the distal end of the filter basket, and the other shaft wire is connected to the proximal end of the filter basket. The actuation handle moves the shaft wires relative to each other to control opening and closing of the basket. In one embodiment, a guidewire-accepting fitting is attached to a distal portion of the device and permits advancement of the device over a guidewire until a desired location along the guidewire is reached. In another embodiment, the basket is provided with a tip resembling a guidewire.

    • A process for providing an endoscopic scissors blade with the necessary roughness to receive a ceramic coating, without first requiring that the blade surfaces be roughened by gritblasting, includes obtaining an investment mold with an interior textured surface. The investment mold is textured by first texturing an injection mold cavity by etching, gritblasting, or electric-discharge machining the inside surface of the injection mold cavity to produce a desired roughness. A blade pattern (wax) is then formed in the injection mold with a resulting textured surface, and using the textured blade pattern, the investment mold with the textured interior cavity is generated. When metal is injected into the investment mold using a lost wax process, the resulting cast blades will have the necessary roughness for permitting a ceramic to be bonded thereto without the need for any additional roughening. Thus, the so-formed blades are coated with a ceramic material. Such scissors blades are used in an endoscopic surgical scissors instrument and are especially useful as the scissor blades of a bipolar cautery endoscopic scissors instrument.

    • An endoscopic suctioning instrument with variable suction is disclosed. The instrument has a hollow cannula having a proximal end and a distal end, and sliding sleeve having a proximal end and a distal end and a longitudinal axis, with the sliding sleeve being coaxial to and closely surrounding and slidingly engaging the hollow cannula. The sliding sleeve has a plurality of lateral holes at its distal end with the lateral holes being spaced along the longitudinal axis. A valve is provided for selectively coupling a suction source to the proximal end of the cannula, and a sleeve actuation mechanism is provided for sliding the sliding sleeve from a first position where the distal end of the sliding sleeve extends beyond the distal end of the cannula such that a first number of the lateral holes are unblocked by said cannula, to a second position where the distal end of the sliding sleeve extends beyond said distal end of the cannula such that fewer lateral holes are unblocked by said cannula, to a third position where the cannula blocks all of said lateral holes of the sliding sleeve. Various embodiments are described with different mechanisms for causing the sleeve to slide, including mechanisms linking the valve to the sleeve.
