    • A laparoscopic apparatus for tunneling dissection including a hollow tunneling member and an inflatable balloon disposed at the distal end of the tunneling member to form together a blunt tipped obturator. The tunneling member has a bore sized allow the insertion of a conventional laparoscope therein to provide observation during surgical procedures. The laparoscope provides direct observation of dissection through the open distal end of the tunneling member. Observation is provided both when the device is used to tunnel between tissue layer to a region of interest within the body of a patient, and during subsequent balloon inflation when the tissue layers are dissected to cause separation of the tissue layers and the creature of an anatomical working space. After the anatomic space has been created, the device and laparoscope are withdrawn through the incision. If needed, a trocar can be inserted into the incision to provide access back to the previously created space where the surgical procedure is to be performed. The anatomic working space may be insufflated with an insufflation gas if required for the particular procedure. The apparatus is prepared for use by inserting the tunneling member into the interior of the balloon forming the balloon into a generally cylindrical roll about the tunneling member, and preferably, through the use of an integral balloon cover, securing the balloon roll to the tunneling member to form a semi-rigid blunt tipped obturator.

    • A surgical instrument facilitates tying of a knot in a length of suture material at a remote surgical location. The instrument is comprised of an inner rod having a length of suture wrapped over the rod in a particular pattern. A needle may be secured to a free end of the suture and the pattern of wrapping the suture on the rod enables loops of suture to move off the rod end and over the needle to form a knot in the suture. The inner rod is received in an interior bore of an outer rod for manually controlled reciprocating movement therein. The inner rod is manually retracted into the interior bore of the outer rod to cause the outer rod to move the loops of suture off the inner rod end and over the needle in forming a knot in the suture. An extendable filament is contained in the inner rod and has a connector at its distal end for holding the free end of the suture. The distal end is curved, so that extending the filament from the inner rod causes it to pass the suture around and behind a tissue to be ligated where the suture may be taken from the filament by a grasper. Pairs of prongs are also provided on the distal ends of the inner and outer rods for sequentially ejecting pre-tied suture knots off the distal end of the inner rod.

    • A sensor that utilizes strips of piezoelectric material to noninvasively measure the surface force/displacement resulting from a blood pressure wave traveling through an artery and transmitted through the arterial wall and overlying tissue, while canceling noise artifact signals is disclosed. Piezoelectric elements create an electrical signal when pressure is applied to their surface. In the preferred embodiment, the sensor is constructed so that there are three sensing elements--a signal sensing element in the center and one noise sensing element on each side of the center element. The center element is placed over an individuals artery, e.g., the radial artery in a persons wrist. When positioned this way, the two noise sensing elements are positioned on each side of the artery.The center element generates a signal that is a function of the pressure wave in the artery, whereas this signal is highly attenuated in the noise sensing elements. However, all three elements detect the noise artifact signals in the general area of the sensor. The area of the noise elements, when combined, is equal to that of the center element. This provides an average of the noise detected by the center element.The signals from the two noise sensing elements are subtracted from the center element signal, thereby canceling the noise in the center element.Also disclosed is a unique method that allows repositioning of the sensor after it has been attached to the patient, a mechanism for maintaining the appropriate hold down pressure, a wrist stabilization device for stabilizing the wrist during monitoring of the blood pressure. The present invention also allows blood pressure calibrations to be obtained using the wrist and arm stabilizer assembly.
