摘要： A surgical fastening apparatus includes a handle, an elongated member extending distally from the handle and a fastener applier and anvil head disposed at the distal end of the elongated member. The elongated member defines a distal offset end portion upon which the fastener applier and the anvil head are mounted. The offset end portion is angularly oriented with respect to the axis of the instrument at a predetermined fixed angle (e.g., either 45°, 60° and 75° relative to the axis of the instrument) thereby positioning the fastener applier and the anvil head at the fixed angle This feature facilitates positioning of the tissue with respect to the instrument and enhances visual access to the tissue when appropriately disposed within the fastener applier and the anvil head. Various mechanisms for effectuating the clamping of the tissue between the fastener holder and anvil, and the firing of the instrument are disclosed.

摘要： An electrosurgical instrument includes a housing having a shaft attached thereto which connects a pair of first and second jaw members such that the jaw members are movable relative to one another to grasp tissue therebetween. The instrument also includes a four-bar handle assembly attached to the housing for actuating a drive rod assembly. The handle assembly includes a handle and a cam-like piston which cooperate to impart a uniform closure pressure against tissue grasped between the jaw members. The instrument also includes a rotating assembly for rotating the jaw members, a knife assembly for separating tissue and a pair of electrical leads connect the jaw members to a source of electrical energy. The electrical leads include slack loops disposed in the rotating assembly which permit rotation of the jaw members about the axis.

摘要： A surgical instrument includes a frame structure defining proximal and distal portions. The proximal portion has an articulating arm for repositionably mounting to a base and the distal portion supporting a stabilizing member. An attachment mounts on the stabilizing member, the attachment being configured and dimensioned such that upon contact with the heart the engaged heart tissue bulges outward. A method of surgery using the attachment includes providing a frame structure defining proximal and distal portions, the proximal portion being repositionably mountable to a base, supporting an attachment on the stabilizing member and engaging heart tissue with the attachment such that the tissue bulges outward to better present the tissue for surgery.
