    • Ureteral stents are tubes which are placed in the ureter between the bladder and kidney to maintain patency when otherwise due to stone burden, invasive tumor, scarring, or whenever the ureter has become obstructed in order to provide drainage of urine from the kidney to the bladder. In the past, stents of this nature are open ended tubes and may allow for the reflux of urine back into the kidney when there is sufficient buildup of pressure within the bladder. Since urine is generally considered to be sterile when within the body, this is generally not a problem. However, if the patient has a bladder infection and/or pyrogens are present, then urine refluxed into the ureter and kidney may result in the development of sepsis in the patient and cause serious illness and even death. The stent of the present invention has a very specific design and avoids this problem by providing a stent having a closed section, i.e., the bladder end section does not contain any fluid passageways and also comprises a means for retaining the stent within the bladder after implantation.

    • A uterine endometrial tissue sample brush 10 simultaneously possesses good ability to exfoliate endometrial tissue without trauma to the uterine canal 38 and good ability to collect the exfoliated endometrial tissue. The brush 10 includes a flexible core 12 having a proximal core end 14 and a distal core end 16, and a cylindrical brush member 18 carried adjacent the distal core end 16. The core 12 is preferably formed from a spirally wrapped pair of wires 22 and includes an atraumatic tip 26 at the distal core end 16. The brush member 18 is about 3 to 5 cm long and has a free outside diameter suited to maintain patient comfort yet comprehensively contact the undilated uterine canal 38, preferably, about 5.08 to 7.62 mm (0.2 to 0.3 in.). The brush member 18 includes a plurality of resilient bristles 20 axially spaced about 0.5 to 1.5 mm apart. A critical aspect of the brush is that the bristles 20 have a stiffness equivalent to that possessed by nylon-6,12 at a diameter of about 0.076 to 0.152 mm (0.003 to 0.006 in.). Such a stiffness unexpectedly gives the brush 10 good exfoliating and collecting abilities simultaneously, in contrast to prior devices which may possess either good exfoliating or good collecting abilities, but not both abilities at the same time. The bristles 20 are preferably composed of nylon-6,12 of about 0.127 mm (0.005 in.) diameter, and are retained between the pair of wires 22 so that they are spirally arrayed. The brush 10 also preferably includes a flexible sheath 24 adapted to slidably receive the core 12 and brush member 18 therein, to prevent the collection of undesired exocervical or vaginal fluid or material during introduction or removal of the brush 10. The sheath 24 preferably has an outside diameter of no more than about 2.8 mm, making the brush 10 especially useful for routinely and repeatedly collecting samples for cancer screening or monitoring ongoing estrogen replacement therapy.

    • A surgical cutting instrument is disclosed in FIG.4 for percutaneously debulking tissue through an access sheath. The cutting instrument includes an inner rotary member tube 401 having a cutting edge 407 at the distal end thereof for cutting and slicing tissue. The outside surface of the rotary inner cutting member tube and outer sheath 409 annularly close about the distal ends thereof for preventing perforation of the surgical tissue bag in which the tissue is contained. The inner cutting member tube includes a hollow passageway for suctioning and aspirating tissue and fluid through the tube and into a collection chamber 419 or out through a vacuum or suction line 422 attached thereto. Vacuum or suction control ports are included at the proximal end of the inner cutting member tube and stationary sheath for also controlling the amount of vacuum or suction applied to the tissue. The surgical cutting instrument includes a control handle 431 for grasping by the physician. A collection chamber is connected to a coupler connected to the stationary sheath for collecting tissue therein. The inner cutting member tube extends through the collection chamber to a drive motor 440 or to a remotely positioned rotary drive mechanism. A vacuum line is attached to the collection chamber for drawing tissue and fluid through the inner cutting member tube and into the collection chamber.

    • A surgical cutting instrument (100) is disclosed for percutaneously debulking tissue through an access sheath commonly used in minimally invasive laparoscopic or endoscopic surgical procedures. The cutting instrument includes an inner rotary member tube (101) having a cutting edge (105) at the distal end thereof for cutting and slicing tissue. The distal end of the cutting member tube extends from the distal end of a stationary sheath (108) which prevents tissue from wrapping around the inner cutting member tube. The inner cutting member tube includes a hollow passageway (104) for suctioning and aspirating tissue and fluid through the tube and into a collection chamber (212) or out through a vacuum or suction line (216) attached thereto. The instrument further includes an outer safety shield (201) having a rounded distal end (203) for preventing the puncture of a surgical tissue bag or from unintentionally cutting tissue by the attending physician. The outer shield has a channel (207) across the distal end through the hollow passageway therein for feeding tissue into the cutting edge and controlling the vacuum applied to the tissue. Vacuum or suction control ports (210) are included at the proximal end of the inner cutting member tube and stationary sheath for also controlling the amount of vacuum or suction applied to the tissue. The surgical cutting instrument includes a control handle (118) for grasping by the physician. A removable collection chamber (212) is connected to a coupler connected to the stationary sheath for collecting tissue therein. The inner cutting member tube extends through the collection chamber to a drive motor (123) or to a remotely positioned rotary drive mechanism. A vacuum line is attached to the collection chamber for drawing tissue and fluid through the inner cutting member tube and into the collection chamber.
