    • A system for delivering low pain cardioversion shocks to the heart wherein the system provides a waveform to the heart that is biphasic and has rounded leading and trailing edges. The rounded leading and trailing edges are believed to decrease the discomfort experienced by the patient. In one embodiment, the circuit has a two capacitors connected in parallel with each other and with an H-bridge. The two capacitors are connected via a switch that can be closed so as to simultaneously charge one capacitor from the other while simultaneously applying voltage to the H-bridge. The circuit also includes a dump resistor that can be connected in parallel with the capacitors so as to increase the rounding of the trailing edges of the waveform. In another embodiment, controllable switches can also be included so as to be able to connect the capacitors in series and apply a sharp peak defibrillation waveform to the heart.

    • An apparatus example comprises a cardiac signal sensing circuit configured to provide a sensed cardiac signal representative of cardiac depolarization events of a subject, a sampling circuit coupled to the cardiac signal sensing circuit, a therapy circuit, and a controller communicatively coupled to the sampling circuit and the therapy circuit. The controller includes a detection module configured to detect tachyarrhythmia using the cardiac signal and a signal analysis module configured to establish significant points (SPs) of the sampled cardiac signal, estimate heart rate during the tachyarrhythmia using the SPs, and provide an indication of whether noise is present in the cardiac signal using the SPs. The controller is configured to select a therapy for delivery by the therapy circuit in response to the tachyarrhythmia detection and to modify the selected therapy according to the heart rate estimation and the noise indication.

    • Systems and methods to indicate heart failure co-morbidity are described. In an example, one or more physiological signals can be analyzed for a specified time period to determine at least one of an indication of a heart failure event or a characteristic of heart failure co-morbidity, such as a characteristic of a non-heart failure physiological event. At least one of the different physiological signals can be compared to a specified criterion indicative of the heart failure event to provide a comparison value, where the comparison value is indicative of whether the at least one of the different physiological signals contributed towards the indication of the heart failure event. An indication of a non-heart failure physiological event can be determined using the heart failure event indication, the non-heart failure physiological event characteristic, and the comparison value indicative of whether the one or more physiological signals contributed towards the heart failure event.
