    • An everting applicator for brachytherapy of body cavities such as the vagina has a flexible sleeve secured to a distal mandrel, both the sleeve and the mandrel having internal lumens. The flexible sleeve has a diverging opening at its distal end, preferably bell-shaped or cone-shaped, such that when the open end is pushed against the mouth of a vagina, the sleeve will evert back upon itself, progressively unrolling to an inside out configuration wherein, fully inserted, the sleeve is fully everted back over the exterior surface of the mandrel. A radiation source, isotopic or electronic, is then inserted into the mandrel lumen to commence a therapeutic irradiation procedure of tissues of the vagina.

    • In brachytherapy radiation treatment, particularly on the breast following a tumor resection, adequate skin to balloon distance must be maintained, to avoid damage to the skin. Often the distance is too small in certain areas or at a single point. The disclosed procedures and devices are effective to manipulate the skin-to-balloon distance to increase that distance when necessary, using vacuum, friction or other means to grip, move and “bunch” the breast tissue from regions of larger balloon-skin distance to regions with inadequate balloon-skin distance. The apparatus can include a ring or ring shaped array that fits around the breast, larger than the balloon, a circular vacuum device, or a cup which is pushed over the breast to move tissue toward a thin tissue region. In many cases only a few millimeters of added distance are needed to meet minimum requirements so that the radiation procedure can commence.

    • A brachytherapy applicator and method of use involve source guides that assume a desired curving, non-linear configuration. A flexible source catheter follows the shape of the source guides when inserted therein. Radiation dose received in various tissue areas can be better controlled using the invention, and the ratio of cavity surface dose to prescription depth dose can be lowered. With sequential manipulation of the source via movement of the catheter, the applicator can deliver radiotherapy to a treatment plan with local variation to prevent overdose, through either stepped or continuous movement of the source. Source guides can be fixed in position and arranged in bowed configuration around a generally central balloon axis, either attached to the balloon wall or not, and the series of off-center guides can be used to shape the dose delivered.

    • A vaginal brachytherapy applicator is versatile to different anatomies and more comfortable to the patient than prior applicators. An inflatable balloon on a shaft may be inelastic or elastic. In one embodiment the shaft is rotatable relative to the balloon for enabling a ring or band of dose delivery, for versatility in radiation placement. Alternatively, the source can be rotated and pulled back within a stationary shaft. An adjustment is provided for reducing the length of balloon that inflates, to accommodate different anatomies and positions. Although isotope radiation sources can be used in the applicator, an electronic source is preferred, and the system and method include procedures for using the applicator with an electronic source.

    • Age-related macular degeneration is treated by radiation delivered from a miniature x-ray tube inserted via a catheter around the globe of the eye, to a position behind the macula. The process can employ an applicator with several parallel guides, inserted around the eye to receive the catheter with x-ray tube (or an isotope) at a matrix of different positions. Methods are described for properly locating the catheter and x-ray tube, using illumination on the catheter and viewing through the front of the eye, or sensors on the catheter and a scanned beam shone from the front of the eye. Fluorescent material excited by x-rays can also be used. Also described are methods and devices for immobilizing the probe once properly located in the eye, for standoff of the x-ray tube from the target tissue, and for achieving prescription radiation dose in the choroid while eliminating dose to adjacent tissues. The x-ray treatment can be enhanced using a radiosensitizing drug, and can be combined with PDT.
